Abstract
A 56-year-old man presented with typical angina pectoris lasting >20 min associated with precordial ST-segment elevation. Urgent coronary angiography showed critical stenosis in the proximal segment of the left anterior descending artery, which resolved with intracoronary nitrate application. He was subsequently diagnosed with hyperthyroidism secondary to exposure of iodinated contrast agent which is thought to be the cause of the coronary spasm. Symptoms resolved upon treatment with propylthiouracil, slow-release diltiazem, isosorbide mononitrate, and aspirin. This unusual case highlights the importance of considering hyperthyroidism in the differential diagnosis of chest pain and coronary artery spasm. We suggest routine thyroid function testing in patients with coronary spasm.
Zusammenfassung
Ein 56-jähriger Mann stellte sich mit einer typischen Angina pectoris vor, die >20 min anhielt und mit einer präkordialen ST-Streckenhebung assoziiert war. Der Patient wurde umgehend einer Koronarangiographie unterzogen. Er wies eine kritische Stenose im proximalen Abschnitt des Ramus interventricularis anterior auf, die durch eine intrakoronare Nitratgabe behoben werden konnte. In der Folge wurde eine sekundäre Hyperthyreose diagnostiziert, die auf einen Kontakt mit jodhaltigem Kontrastmittel zurückging und als Auslöser des Koronarspasmus anzusehen ist. Der Patient wurde mit Propylthiouracil, retardiertem Diltiazem, Isosorbidmononitrat und Aspirin behandelt, woraufhin die Symptome verschwanden. Dieser ungewöhnliche Fall unterstreicht, wie wichtig es ist, eine Hyperthyreose in der Differenzialdiagnose von Thoraxschmerzen und koronararteriellen Spasmen in Betracht zu ziehen. Wir empfehlen eine routinemäßige Untersuchung der Schilddrüsenfunktion bei Patienten mit Koronarspasmus.
