Abstract
Background and Objective:
Some patients with open bite who have been treated with combined fixed orthodontic appliances and bimaxillary osteotomy relapse in the vertical dimension after the end of treatment following initially successful correction of the vertical dimension. The aim of this study was to identify those parameters that raise the tendency to relapse.
Materials and Methods:
Nineteen patients who underwent combined treatment using fixed orthodontic appliances and bimaxillary osteotomy due to open bite (overbite < 0 mm), were examined and documented at least 2 years after surgery. Pre- and post-orthodontic and pre- and post-operative study models and lateral head films were analyzed. Overbite reduction after the end of treatment was considered as relapse.
Results:
In our study cohort, the appearance of relapse in the vertical correlated with the enlargement of Björk’s sum angle (odds ratio 10.7) and Jarabak’s ratio (odds ratio 7) by means of surgery, as well as with dysfunctional habits persisting after treatment such as mouth breathing (odds ratio 12.5) and a visceral swallowing pattern (odds ratio 8.3). The existence at baseline of isolated anteinclination of the anterior maxilla or isolated steep inclination of the mandible also made vertical relapse more probable (odds ratio 7.5).
Conclusion:
Particularly deserving of attention during the combined interdisciplinary treatment of patients with open bite are the achieved reduction in posterior facial height by surgical posterior impaction of the maxilla, as well as correction of dysfunctional parameters during swallowing and breathing.

Hintergrund und Ziel:
Nach zunächst erfolgreicher Einstellung der Vertikalen bei Patienten mit offenem Biss durch die kombinierte Behandlung mit Multibracketapparatur und bimaxillärer Umstellungsosteotomie kommt es bei einigen Patienten nach Abschluss der Behandlung zum Rezidiv in der Vertikalen. Es sollten die Parameter, die das Auftreten eines Rezidives begünstigen, eruiert werden.
Patienten und Methodik:
Neunzehn Patienten mit anfänglich offenem Biss (Overbite < 0 mm), die eine kombinierte kieferorthopädisch-kieferchirurgische Behandlung mit bimaxillärer Umstellungsosteotomie erhalten hatten, konnten mindestens 2 Jahre nach der Operation kontrolliert und dokumentiert werden. Modelle und FRS-Bilder vom Anfang und Ende der kieferorthopädischen Behandlung sowie prä- und postoperativ wurden analysiert. Eine Verringerung des Überbisses nach dem Behandlungsende wurde als Rezidiv gewertet.
Ergebnisse:
Das Auftreten eines Rezidivs in der Vertikalen hing in der vorliegenden Patientengruppe von der Vergrößerung des Summenwinkels nach Björk (Odds Ratio 10,7) und der Ratio nach Jarabak (Odds Ratio 7) durch die Operation sowie den bei der Nachkontrolle noch bestehenden funktionellen Faktoren Mundatmung (Odds Ratio 12,5) und viszerales Schluckmuster (Odds Ratio 8,3) ab. Auch das ursprüngliche Vorliegen einer isolierten anterioren Inklination der Maxilla oder einer isolierten posterioren Inklination der Mandibula machten das Auftreten eines Rezidives in der Vertikalen wahrscheinlicher (Odds Ratio 7,5).
Schlussfolgerung:
Bei der interdisziplinären Behandlung der Patienten mit offenem Biss sollte neben der etablierten operativen Verkürzung der hinteren Gesichtshöhe durch posteriores Impaktieren der Maxilla auch besonders auf die Korrektur der funktionellen Parameter Schluckmuster und Atemmodus geachtet werden.
