Abstract
Background
Intraabdominal hemorrhagic cystic masses may be of benign or malignant origin. Differential diagnoses include duplication cysts, pseudocysts, gastrointestinal stromal tumors (GIST), and various infectious diseases, amongst many others.
Case report
A 40-year-old male with no relevant past medical history presented to the emergency room with acute right abdominal pain. His white blood cell (WBC) count was elevated and CT scan showed a 10-cm cystic mass in the right upper quadrant lodged between duodenum and hepatic flexure, causing obstruction of the ascending colon which was significantly dilated. Carcinoembryonic antigen (CEA) and carbohydrate antigen (CA)19‑9 were negative and colonoscopy showed a submucosal mass obstructing the hepatic flexure that could not be passed. Endoscopic ultrasound showed a large fluid-filled cyst. Due to poor bowel preparation, no attempts were made to aspirate fluid. He underwent laparoscopic right hemicolectomy with resection of the cyst. The patient recovered well from surgery and was symptom and recurrence free after 1 year. Pathology revealed a hemorrhagic cyst.
Conclusion
It was not possible to establish a definite cause of the hemorrhagic cyst. The patient denied any trauma and had no symptoms preceding the acute hemorrhage. Such lesions should be surgically removed; laparoscopy is a good option in similar cases.

Abdominelle hämorrhagische Zysten können gutartig und bösartig sein. Differenzialdiagnosen inkludieren Duplikationszysten, Pseudozysten, gastrointestinale Stromatumoren (GIST), verschiedene Infektionen und viele andere Erkrankungen.
Kasuistik
Ein 40-jähriger gesunder Mann ohne relevante medizinische Vorgeschichte wurde in der Notfallaufnahme mit akuten Schmerzen im rechten Oberbauch vorstellig. Im Labor fand sich eine Leukozytose, und in der Abdomen-CT zeigte sich eine 10 cm große zystische Struktur zwischen Duodenum und rechter Flexur. Diese verursachte eine Obstruktion des aufsteigenden Kolons, das deutlich dilatiert war. Karzinoembryonales Antigen (CEA) und CA19‑9 („carbohydrate antigen 19-9“) waren normal. In der Koloskopie zeigte sich eine submuköse Raumforderung, welche eine Striktur verursachte, die nicht passierbar war. Der endoskopische Ultraschall ergab eine große Zyste, auf eine Biopsie wurde wegen schlechter Darmvorbereitung verzichtet. Eine laparoskopische rechtseitige Hemikolektomie mit Entfernung der Zyste im Mesenterium wurde durchgeführt. Der Patient hatte einen komplikationslosen Verlauf und wies nach einem Jahr keinen Hinweis auf ein Rezidiv auf. Die pathologische Untersuchung ergab eine simple hämorrhagische Zyste.
Schlussfolgerung
Es ließ sich keine klare Ursache für die Zyste finden. Der Patient verneinte die Frage nach einem Trauma vor der Einblutung und wies vorher keine Symptome auf. Generell sollten diese Zysten chirurgisch entfernt werden. Dies sollte, wenn möglich, laparoskopisch erfolgen.
