Zusammenfassung

Rektale Lymphome stellen eine sehr seltene und zugleich heterogene Krankheitsgruppe dar. Demzufolge liegen kaum Daten zum diagnostischen Vorgehen und zur Behandlungsstrategie vor. Entscheidend für das klinische Management sind histologischer Typ und Stadium des Lymphoms. Auf dieser Basis reicht die Spannbreite der Behandlung von „watch-and-wait“ über lokale Maßnahmen (endoskopische Resektion, Bestrahlung, Operation) bis zur Immunchemotherapie und im Einzelfall einer konsekutiven Stammzelltransplantation. Auch die Kombination dieser Modalitäten stellt eine Option dar. Die Nachsorge orientiert sich an dem Therapieziel und den möglichen Optionen für eine Zweitlinientherapie bei Rezidiv oder Persistenz des Lymphoms.

Abstract

Rectal lymphomas are very rare and also represent a heterogeneous group of diseases. As a result, data on the diagnostic approach and treatment strategy are mostly lacking. The histological type and stage of the lymphoma are the two decisive criteria for the clinical management. Based on this the therapeutic range extends from a watch and wait strategy to local measures (endoscopic resection, irradiation, surgery) and immunochemotherapy and in individual cases a consecutive stem cell transplantation. The combination of these modalities is also an option. The follow-up is determined by the therapeutic target and potential options for second-line treatment in cases of relapse or persistence of the lymphoma.
