Zusammenfassung
Vor 40 Jahren entwickelte Gerhard Bueß ein damals völlig neues System aus einem Operationsrektoskop mit einführbarer stereoskopischer Optik, in die eine Zusatzoptik für eine Videokamera eingeführt wird, einem Set von speziellen Instrumenten und einem Haltearm für die Befestigung am Op.-Tisch. Dazu gehörten neben einer Kaltlichtquelle auch ein Gerät zur CO2-Insufflation in das Rektum, das durch eine kontinuierlichen Druckmessung das Lumen permanent dilatiert halten kann. Mit diesem System wurden bei guter Übersicht lokale Tumorexzisionen im gesamten Enddarm möglich. Die anspruchsvolle Operationstechnik machte die Entwicklung eines Kurses notwendig, der von Chirurgen aus zahlreichen Ländern absolviert wurde. Durch die erfolgreiche Einführung und Weiterverbreitung der Methode sowie durch technische Weiterentwicklungen wurden in diesen vier Jahrzehnten zahllose lokale endoskopische Eingriffe möglich. Gleichzeitig entstanden in aller Welt Arbeitsgruppen, die eine große Zahl an Publikationen veröffentlichten. Diese enthielten einen ausführlichen Diskurs aller technischen und onkologischen Fragen, welche die naturgemäß begrenzten Möglichkeiten von lokalen Exzisionen mit sich bringen. Mit den immer besser werdenden Möglichkeiten der präoperativen Diagnostik werden auch die Indikationsstellungen immer präziser, und die Methode kann ihren Stellenwert bei der Behandlung von rektalen Adenomen und Frühstadien von Malignomen beibehalten.
Abstract
Some 40 years ago Gerhard Bueß developed a then completely new system consisting of a surgical rectoscope with insertable stereoscopic optics into which an additional optic for a video camera was inserted, a set of special instruments, and a holding arm for attachment to the operating table. In addition to a cold light source, this also included a device for CO2 insufflation into the rectum, which can keep the lumen permanently dilated through continuous pressure measurement. With this system local tumor excision in the entire rectum was possible with a good overview. The demanding surgical technique made it necessary to develop a dedicated training course that was completed by surgeons from numerous countries. Countless local endoscopic interventions have become possible over the past four decades due to the successful introduction and dissemination of the method as well as technical advances. At the same time, working groups emerged all over the world, which published a large number of publications. These included a detailed discourse on all technical and oncological issues that the naturally limited possibilities of local excisions entail. With the continuously improving possibilities of preoperative diagnostics, the indications are also becoming more and more precise and the method can retain its value in the treatment of rectal adenomas and early stages of malignancies.
