Zusammenfassung
Die laparoskopische Chirurgie ist ein elementarer Bestandteil der gesamten Abdominalchirurgie. Dennoch hat sich bis heute nur die laparoskopische Cholezystektomie als Goldstandard durchgesetzt und wird bei etwa 90 % der Patienten weltweit erfolgreich eingesetzt. Bei anderen abdominellen Operationen ist der Anteil an minimal-invasiver Chirurgie bedeutend geringer. Die laparoskopische kolorektale Chirurgie – v. a. bei onkologischer Indikation – hat sich seit ihrer Erstbeschreibung 1991 nur zögerlich verbreitet, wobei im internationalen Vergleich große regionale Unterschiede bestehen. Auch wenn ein Großteil der Patienten mit einem kolorektalen Karzinom (CRC) für eine laparoskopische Operation geeignet wäre, liegt die Verbreitung vielerorts noch unter 50 % der Eingriffe. Die allgemeinen Vorteile der laparoskopischen Chirurgie im früh-postoperativen Verlauf sind bekannt. Zahlreiche Studien haben auch zumindest die Gleichwertigkeit der laparoskopischen Operation beim Karzinom in Bezug auf die onkologischen Kriterien nachgewiesen. In den deutschen Leitlinien wird die laparoskopische Operation beim CRC bei entsprechender Expertise des Behandlungsteams seit 2013 als Alternative zur konventionellen Operation betrachtet. Für eine weitere Implementierung der laparoskopischen kolorektalen Chirurgie sind eine Standardisierung der Operationstechnik, eine profunde Ausbildung und Training des Chirurgen sowie eine adäquate technische Ausstattung die wichtigsten Faktoren, die von der Lehre bis zur einzelnen Klinik umgesetzt werden müssen.
Abstract
Laparoscopic surgery is an elementary component of the complete abdominal surgery. Nevertheless, to date only laparoscopic cholecystectomy has become established as the gold standard and is used successfully in around 90% of patients worldwide. For other abdominal operations a much lower proportion of minimally invasive surgery applies. Since it was first described in 1991, laparoscopic colorectal surgery, especially for oncological indications, has spread only hesitantly although in an international comparison there are large regional differences. Even if the majority of patients with colorectal cancer would be suitable for laparoscopic surgery, in many places the prevalence is often less than 50%. The general advantages of laparoscopic surgery in the early postoperative period are well known. Numerous studies have also demonstrated that laparoscopic surgery for carcinomas is at least equivalent in terms of oncological criteria. In the German guidelines laparoscopic surgery for colorectal cancer has been considered an alternative to conventional surgery since 2013 if the treatment team has the appropriate expertise. For further implementation of laparoscopic colorectal surgery, standardization of the surgical technique, thorough education and training of surgeons and adequate technical equipment are the most important factors that must be implemented from the teaching to the individual clinic.
