Zusammenfassung
Hintergrund
Rektovaginale Fisteln (RVF) sind für die betroffenen Patientinnen häufig mit einer starken Einschränkung der Lebensqualität verbunden. Einen Goldstandard in Bezug auf die Therapie von RVF gibt es nicht. Derzeit gibt es verschiedenste Operationsverfahren, die sich nicht nur durch den Zugangsweg, sondern insbesondere durch Faktoren wie Invasivität und Rezidivrate unterscheiden.
Ziel der Arbeit
Nach Review der Literatur erfolgt die Vorstellung aktueller Therapieoptionen für RVF.
Methoden
Bei der Wahl des Therapieverfahrens handelt es sich um eine individuelle Entscheidung, welche die Situation und das Krankheitsbild der jeweiligen Patientin berücksichtigt. Fistelgröße, -ursache und -verlauf sowie das Ausmaß eines möglichen Sphinkterdefekts haben Einfluss auf die Wahl des Therapieverfahrens und das postoperative Outcome.
Ergebnisse
Zu den etablierten Operationsverfahren von RVF zählt die Fistelexzision mit primärem Verschluss bzw. plastischer Deckung teilweise unter Einsatz von Bionetzen. Bei komplizierten Fisteln ist die Interposition von Gewebe (z. B. Grazilis- oder Martius-Plastik) eine Option, um die Rezidivrate zu senken. Ein transabdomineller Eingriff ist bei sehr hohen Fisteln indiziert. Versuche zur minimal-invasiven Therapie von RVF sind Gegenstand von Studien. Die Evidenz ist hier noch ungenügend.
Diskussion
Ein Problem der Therapie von RVF ist die hohe Rezidivrate. Die anale Kontinenz ist nach mehreren invasiven Fisteloperationen klar gefährdet. Operative Eingriffe bei RVF sollten daher in einem Zentrum mit entsprechender Expertise durchgeführt werden.
Abstract
Background
Rectovaginal fistulas (RVF) are often associated with a severe impairment of the quality of life for the affected patients. There is no gold standard with respect to the treatment of RVF. There are currently a wide variety of surgical procedures that differ not only in the access route but also in factors such as invasiveness and recurrence rate.
Objective
After a review of the literature, the current treatment options for RVF are presented.
Methods
The choice of the treatment procedure is an individual decision that takes the situation and the clinical picture of the respective patient into account. The size, cause and course of the fistula as well as the extent of a possible sphincter defect influence the choice of treatment method and the postoperative outcome.
Results
One of the established surgical procedures of RVF is fistula excision with primary closure or plastic reconstruction, sometimes with the use of biologic meshes. In complicated fistulas, the interposition of tissue (e.g. gracilis muscle or Martius flap) is an option to reduce the recurrence rate. Transabdominal surgery is indicated for very high fistulas. Attempts with minimally invasive treatment for RVF are the subject of studies. The evidence is still insufficient.
Discussion
A problem with RVF treatment is the high rate of recurrence. The anal continence is clearly endangered after several invasive fistula operations. Operative interventions for RVF should therefore be carried out in a center with the appropriate expertise.
