Zusammenfassung
Hintergrund
Die operative Therapie der rektovaginalen Fisten (RVF), insbesondere komplexer Fisteln, stellt nach wie vor eine Herausforderung dar. Es gibt verschiedene Operationstechniken, von denen sich jedoch keine als Methode der Wahl durchgesetzt hat.
Ziel der Arbeit
Die gängigen Therapieverfahren bei RVF werden dargestellt.
Methoden
Die Wahl des Therapieverfahrens richtet sich nach der Klassifikation der Fistel (Größe, Lokalisation, simpel/komplex), nach der Sphinkterfunktion, den Gewebeverhältnissen sowie dem Krankheitsverlauf (erstmalige Operation oder Rezidiv). Nicht selten ist die vorübergehende Anlage eines Diversionsstomas erforderlich, um die Bedingungen zur Ausheilung der Fistel zu begünstigen.
Ergebnisse
Das postoperative Outcome ist u. a. abhängig von der Fistelursache. Fistelrezidive sind oft mit Morbus Crohn assoziiert. Andere prognostisch ungünstige Faktoren können zudem die unzureichende Durchblutung im Narbengewebe nach stattgehabter Fisteloperation, die bakterielle Besiedlung des rektovaginalen Bereichs, Nikotinabusus oder eine chronische Gewebsischämie durch vorausgegangene Radiotherapie sein.
Diskussion
Unabhängig von der Wahl des Operationsverfahrens sollten die wichtigsten technischen Aspekte eine weite Dissektion um die Fistel, die vollständige Exzision des Fisteltraktes, eine ausreichende Mobilisation des umgebenden Gewebes, das Entfernen von Narbengewebe und eine spannungsfreie Naht sein.
Abstract
Background
Surgical treatment of rectovaginal fistulas (RVF) continues to be a challenge, particularly for complex fistulas. There are various surgical techniques, none of which has become established as the method of choice.
Objective
The commonly used treatment procedures for RVF are presented.
Methods
The choice of surgical procedure depends on the classification of the fistula (size, localization, simple/complex), the sphincter function, the tissue conditions and the course of the disease (initial operation or recurrence). The temporary placement of a diverting stoma is frequently necessary in order to promote the conditions for healing the fistula.
Results
The postoperative outcome is dependent on the cause of the fistula. Fistula recurrences are more commonly associated with Crohn’s disease. In addition, other prognostically unfavorable factors can be insufficient perfusion of the scar tissue after surgical treatment of a fistula, bacterial colonization of the rectovaginal area, nicotine abuse or chronic tissue ischemia due to previous radiotherapy.
Conclusion
Regardless of the surgical procedure chosen, the most important technical aspects should be extensive dissection around the fistula, complete excision of the fistula tract, adequate mobilization of the surrounding tissue, removal of scar tissue and a tension-free suture.
