Zusammenfassung
Die laparoskopische Chirurgie hat sich in den vergangenen 100 Jahren Schritt für Schritt weiterentwickelt. Die laparoskopische Cholecystektomie gilt als Goldstandard und wird bei ca. 90 % der Patienten weltweit erfolgreich eingesetzt. Im Vergleich dazu hat sich die laparoskopische kolorektale Chirurgie seit ihrer Erstbeschreibung 1991 nur sehr zögerlich entwickelt, v. a. aus der frühen Sorge einer unzureichenden onkologischen Resektion. Auch wenn 70–80 % der Patienten mit einem kolorektalen Karzinom für eine laparoskopische Operation geeignet wären, liegt die Verbreitung weltweit bei 30–50 % der Eingriffe. Zahlreiche Studien haben die Gleichwertigkeit der laparoskopischen Operation auch beim Karzinom nachgewiesen, wobei sich die Vorteile in der Regel auf die – bekannten – Vorteile der minimal-invasiven Chirurgie im früh-postoperativen Verlauf beschränken. Auch in den Leitlinien wird die laparoskopische Operation bei entsprechender Expertise des Behandlungsteams als Alternative zur konventionellen Operation betrachtet. Eine Standardisierung der Operationstechnik, eine profunde Ausbildung und Training des Chirurgen sowie eine adäquate technische Ausstattung sind die wichtigsten Faktoren, um die weitere Verbreitung der laparoskopischen kolorektalen Chirurgie sicher und nachhaltig zu gewährleisten.
Abstract
Over the last 100 years laparoscopic surgery has progressed step by step. Laparoscopic cholecystectomy is considered to be the gold standard and is successfully used in approximately 90% of patients worldwide. In comparison, laparoscopic colorectal surgery has developed very slowly since its initial description in 1991, especially due to the early worries of inadequate oncological resection. Even if 70–80% of patients with colorectal carcinoma would be suitable for laparoscopic surgery, only 30–50% worldwide are treated laparoscopically. Numerous studies have shown the equivalency of laparoscopic surgery also for carcinomas, whereby the advantages are usually limited to the known advantages of minimally invasive surgery in the early postoperative course. In the guidelines the laparoscopic operation is also considered as an alternative to conventional surgery with appropriate expertise of the treatment team. A standardization of the surgical technique, a profound education and training of the surgeon as well as adequate technical equipment are the most important factors to ensure the continued spread of laparoscopic colorectal surgery in a safe and sustainable manner.
