Hintergrund
Die endoskopische Resektion (ER) ist die Therapie der Wahl bei benignen Kolonpolypen. Auch für ausgewählte kolorektale Frühkarzinome stellt die ER eine kurative Therapieoption dar.
Fragestellung
Aktuelle Therapieempfehlungen und publizierte Ergebnisse der Diagnostik und Therapie beim kolorektalen Frühkarzinom werden dargestellt.
Material und Methoden
Die aktuelle Literatur wird ausgewertet und anhand von endoskopischen Fallbeispielen erläutert.
Ergebnisse
Neue Technologien wie das „narrow band imaging“ (NBI) haben die endoskopische Diagnostik und Dignitätsbeurteilung gastrointestinaler Frühneoplasien verbessert. Die Entwicklung der endoskopischen Submukosadissektion (ESD) hat durch eine verbesserte R0-Resektionsrate und ein vermindertes Rezidivrisiko die Rate kurativer endoskopischer Resektionen erhöht. Resektionskriterien für kolorektale Frühkarzinome sind in nationalen und internationalen Leitlinien festgelegt. Für kolorektale Frühkarzinome gilt aktuell eine Submukosainvasion von 1000 µm als Grenze der endoskopischen Resektion. Möglicherweise kann diese Grenze für bestimmte Subgruppen ohne weitere Risikofaktoren erweitert werden; hierzu sind weitere Daten abzuwarten. Für benigne Kolonpolypen stellt die endoskopische Mukosaresektion (EMR) ein effektives Resektionsverfahren dar. Endoskopische Kontrolluntersuchungen sind zur Erkennung und erneuten ER von Rezidiven obligat.
Schlussfolgerung
Es gibt erhebliche Fortschritte in der endoskopischen Diagnostik und Therapie kolorektaler Frühkarzinome. Weitere Studien sind erforderlich, um die Möglichkeiten und Grenzen der endoskopischen Resektion durch eine verbesserte Datenlage noch genauer zu definieren.
Abstract
Background
Endoscopic resection (ER) is the treatment of choice for benign colorectal polyps. In selected cases it even provides a minimally invasive curative treatment option for early colorectal cancer.
Objective
Presentation of current guideline recommendations and discussion of published data regarding the diagnostics and endoscopic treatment of early colorectal cancer.
Material and methods
Analysis of the current literature and presentation of endoscopic case reports.
Results
New technologies, e. g. narrow band imaging (NBI) have improved the endoscopic diagnostics and assessment of the dignity of early gastrointestinal neoplasms. The development of endoscopic submucosal dissection (ESD) allows improved R0 resection rates and minimization of the recurrence risk leading to higher rates of curative endoscopic resections. Resection criteria for early colorectal cancer are defined in national and international guidelines. In early colorectal cancer submucosal invasion exceeding 1000 µm is defined as the limit for ER in current guidelines. This threshold might be too strict for subgroups without further risk factors and further data are awaited. Endoscopic mucosal resection (EMR) is an effective treatment option for benign colorectal adenomas and surveillance endoscopy is mandatory for detection and renewed ER of recurrences.
Conclusion
Substantial progress has been made in the endoscopic diagnostics and treatment of early colorectal cancer; however, further studies are urgently needed for a better definition of the possibilities and limitations of ER.
