Zusammenfassung
Ein 34-jähriger Mann mit M.-Crohn-Anamnese wurde wegen starker Schmerzen im rechten Oberschenkel in unsere stationäre Behandlung aufgenommen. Auf der Computertomographie von Becken und Oberschenkeln in koronaler Ebene waren Gaseinschlüsse im Weichteilgewebe erkennbar, sie erstreckten sich nach inferior an den rechten Femoralgefäßen entlang in den rechten Oberschenkel. Drei Tage nach Aufnahme wurde ein protektives Kolostoma angelegt, mehrere Inzisionen in den Oberschenkel wurden vorgenommen und gespült sowie drainiert. Der Patient erholte sich gut von dem Eingriff. Diese Kasuistik zeigt, dass sich eine intraabdominelle Sepsis bei M. Crohn als extraabdomineller Abszess manifestieren kann. Für diese Komplikation kann ein frühes und aggressives Management angezeigt sein.
Abstract
A 34-year old man with a history of Crohn’s disease was admitted to our hospital with severe pain of the right thigh as the predominant symptom. A coronal section of a CT scan of the pelvis and thigh demonstrated gas in the soft tissues extending inferiorly along the right femoral vessels into the right thigh. Diverting colostomy, multiple incisions of the thigh with sufficient irrigation and drainage were performed 3 days after admission. The patient recovered well from the operation. This case illustrates that in Crohn’s disease intra-abdominal sepsis may present as an extra-abdominal abscess. Early aggressive management may be indicated for this condition.
