Zusammenfassung
Hintergrund:
Die Diagnostik perirektaler Raumforderungen ist computertomographisch und kernspintomographisch bei ausreichender Kontrastmittelfüllung des Rektums mit einer Treffsicherheit von > 90% möglich. Eine sichere Abgrenzung zum Rektum oder zu den inneren Genitalien ist jedoch schwierig. Häufig können exakte Aussagen ebenso wie Angaben zur Dignität nicht getroffen werden.
Methodik:
Im Rahmen der transrektalen Ultraschalldiagnostik diagnostizierten wir in zwei Fällen pelvine Metastasen von Bronchialkarzinomen mit Infiltration der Rektumwand. Die histologische Sicherung der Befunde erfolgte durch die ultraschallgestützte Punktion. Das diagnostische Vorgehen wird beschrieben.
Ergebnisse:
Diese Berichte dokumentieren das seltene Auftreten von Metastasen des Bronchialkarzinoms im kleinen Becken und in den Weichteilen des Beckens. Die seltene Metastasierung des Bronchialkarzinoms in die Beckenorgane sollte bekannt sein.
Fazit:
Die transrektale 3D-Endosonographie hat in der Diagnostik pararektaler Raumforderungen und pelviner Metastasen mit dem Vorteil der transrektalen ultraschallgestützten Punktion in Abhängigkeit von der Erfahrung des Untersuchers einen wesentlichen diagnostischen Stellenwert.
Abstract
Background:
The diagnosis of perirectal masses can be accomplished by computer tomography and magnet resonance imaging if the rectum is contrasted. The accuracy of these methods is > 90%. But often the assessment of the exact borders, with respect to the rectum wall and to the genitalia, is difficult.
Methods:
We report about two cases of pelvic metastasis and rectal infiltration of bronchial carcinoma detected by ultrasound-guided, transrectal aspiration biopsy. The diagnostic algorithm is described.
Results:
The reports describe the rare occurrence of pelvic metastasis of bronchial carcinoma. This localization of bronchial carcinoma metastasis should also be known.
Conclusion:
Transrectal 3-D-ultrasound is very important in the diagnosis of perirectal masses and pelvic metastasis and enables transrectal ultrasound-guided biopsy. Therefore transrectal 3-D-ultrasound is of major importance in the preoperative staging and postoperative follow-up of rectum cancer and allows the diagnosis of pararectal findings using the ultrasound-guided, transrectal aspiration.
