Zusammenfassung

Die Anlage von temporären oder definitiven Stomata ist ein wichtiger Aspekt in der kolorektalen Chirurgie. Zur Vermeidung von Komplikationen bedarf es einer guter präoperativen Planung, einer präzisen Operationstechnik und einer professionellen postoperativen Betreuung, am besten mithilfe von speziell ausgebildeten Stomatherapeuten. Dennoch sind Komplikationen, etwa peristomale Hernien, Prolaps, Stenosen etc. im Langzeitverlauf verhältnismäßig häufig und bedürfen einer suffizienten konservativen und/oder operativen Therapie. Wichtig sind auch Kenntnisse über die normale bzw. pathologische Stomaphysiologie sowie über die modernen Versorgungsmöglichkeiten eines Anus praeter naturalis. Ebenso werden psychosoziale Aspekte diskutiert und Kontaktadressen für Selbsthilfegruppen aufgeführt.

Abstract

The construction of a temporary or definite stoma continues to be an important issue in colorectal surgery.To avoid complications, stomas need to be carefully planned preoperatively which is best achieved with the aid of a trained stoma therapist. Furthermore, the operative technique needs to be of highest standards and all patients should have access to competent postoperative stoma care. Nevertheless, stoma complications, such as peristomal herniation, prolapse, stenosis, etc. do occur quite frequently and need to be managed either conservatively or surgically. Aspects of normal and pathological stoma physiology as well as modern stoma appliances are also discussed. Furthermore, psycho-social aspects are touched on and a list of self-help groups is specified.
