Primary Adenocarcinoma of the Ileostomy Following Total Colectomy for Ulcerative Colitis

Abstract

Background:

Patients with ulcerative colitis are at high risk for cancer and are often treated by total proctocolectomy with ileostomy. Primary adenocarcinoma arising at an ileostomy site is an exceedingly rare complication of long-term ileostomies.

Patient and Methods:

We add the 30th case to the literature, a patient 47 years after colectomy and ileostomy for ulcerative colitis.

Conclusions:

The pathogenetic mechanism for the malignant transformation in ileostomies is not known and might be different in patients with ileostomy for ulcerative colitis or for familial adenomatous polyposis.We discuss the hypothesis of inflammation-dysplasia-carcinoma sequence due to colonic metaplasia or due to preoperative backwash ileitis in patients with ulcerative colitis and long-term ileostomies.

Hintergrund:

Patienten mit Colitis ulcerosa gehören einer Krebsrisikogruppe an. Die Therapie besteht oft in einer totalen Kolektomie mit endständigem Ileostoma. Die Karzinombildung an einem Ileostoma ist eine äußerst seltene Komplikation von Langzeitstomatas.

Patient und Methode:

Wir berichten über eine Patientin mit primärem Adenokarzinom des Ileostomas 47 Jahre nach totaler Kolektomie bei Colitis ulcerosa.

Schlussfolgerungen:

Der pathogenetische Mechanismus, der zur malignen Entartung im Ileostoma führt, ist unklar und möglicherweise unterschiedlich bei Patienten mit Colitis ulcerosa oder familiärer Polyposis coli.Wir diskutieren die Hypothesen der “colonic metaplasia“ der Stomaschleimhaut sowie der „backwash ileitis“ als Ursache der Karzinomentstehung.
