Zusammenfassung
Die Kompressionsmarknagelung kombiniert das Prinzip der intramedullären Osteosynthese mit der Möglichkeit, durch interfragmentäre Kompression eine erhöhte Primärstabilität zu erzielen. Durch die daraus zusätzlich resultierende Schmerzreduktion können eine sehr rasche Vollbelastung angeschlossen und eine verkürzte Knochenbruchheilung erzielt werden. Idealer Frakturtyp ist der Quer- oder kurze Schrägbruch. Des Weiteren kann das Kompressionsprinzip zur gesicherten Fragmentadaption auch bei instabileren Frakturen zur Anwendung kommen. Auf diese Weise kann eine Fragmentdehiszenz sicher vermieden werden. Ideales Einsatzgebiet für die Kompressionsmarknagelung ist die rekonstruktive Chirurgie (Pseudarthrosenbehandlung, Korrekturosteotomie, Arthrodese). Mit biomechanischen Methoden kann der Stabilitätsgewinn nachgewiesen werden, wobei interfragmentär Kräfte bis zu 4000 N auftreten. Die hier vorgestellten Implantate können sowohl antegrad als auch retrograd mit demselben Instrumentarium eingebracht werden.
Abstract
Interlocking compression nailing combines the principle of intramedullary osteosynthesis and the possibility of interfragmentary compression, which results in a high level of primary stability and less pain, allowing early full weightbearing. This can mean a shorter hospital stay and faster bone healing. Transverse and short oblique fractures are the ideal indication for this technique. The compression principle can also be useful to ensure fragment adaptation in more unstable fracture types and has proved effective in prevention of fracture diastasis. Compression nailing is ideal in reconstructive surgery such as is performed for treatment of nonunions, corrective osteotomy, and arthrodesis. The improved stability can be demonstrated by biomechanical methods; interfragmentary forces of up to 4000 N have been observed. Antegrade and retrograde techniques can be used for insertion of the nails presented in this paper with no need for different instrumentation setups.
