Zusammenfassung
In Hinsicht auf die Entscheidung, ob eine Schultereckgelenkverletzung konservativ oder operativ behandelt werden soll, finden sich in der Literatur diskrepante Aussagen eigentlich ausschließlich bezüglich der Klassifikationstypen Tossy III bzw. Rockwood III und V. AC-Gelenksprengungen entsprechend Tossy I und II bzw. Rockwood I und II werden nach allgemeiner Einschätzung konservativ, Rockwood IV und VI operativ und Rockwood V überwiegend operativ therapiert. Die genaue Analyse der vorliegenden Vergleichsstudien weist aus, daß in keiner Arbeit ein Vorteil der konservativen Therapie im Vergleich zur operativen Verfahrensweise bei den Rockwood-III-Verletzungen schlüssig nachgewiesen werden konnte. Aus diesem Grund favorisieren wir bei dieser Verletzungsform nach wie vor die operative Therapie im Sinn der relativen Indikation bis zum 50. Lebensjahr, zumindest bei hohem beruflichen oder sportlichen Aktivitätslevel. Die Rockwood-Klassifikation erweist sich im Vergleich zur Tossy-Klassifikation als geeigneter, weil sie eine Unterscheidung zwischen absoluten (IV, VI und V) und relativen (III) Indikationen zuläßt.
Abstract
Concerning the decision on whether acromioclavicular (AC) dislocations should be treated conservatively or operatively, only classification types Tossy III respectively Rockwood III and V are a point at issue in the literature. Tossy/Rockwood I and II dislocations are generally treated conservatively, while Rockwood IV, VI, and, in our opinion, also Rockwood V require surgery. In none of the papers reviewed here could a concrete advantage be found in conservative treatment of type III as compared to operative treatment. For this reason, we continue to prefer surgery for treating Rockwood III injuries. We see a relative indication in patients below the age of 50 years, at least those with above average activity levels. In comparison, the Rockwood classification proves better suited, as it distinguishes between absolute (IV, VI, and V), and relative (III) indications.
