Weichteilrekonstruktion bei komplexem Fußtrauma

Zusammenfassung

Defekte der Fußsohle und des Übergangsgebiets zum Fußrücken sollten nach ausgedehntem, rechtzeitigem Débridement einer suffizienten, statisch belastbaren, plastischen Deckung zugeführt werden. Zur Rekonstruktion der Fußsohle bieten lokale, mikrochirurgisch gehobene, gefäßgestielte Insellappenplastiken die erste Wahl. Sie sind in dem gleichen, gut vaskularisierten Gewebeaufbau frühzeitig suffizient belastbar. Sämtliche der genannten lokalen Lappenplastiken setzen eine gute Vaskularisation des Fußes als Endstrombahngebiet voraus. Große, fußrandübergreifende Gewebedefekte bei schwer traumatisierter Extremität bilden in der Indikationsstellung eine Domäne der mikrovaskulären freien Gewebetransplantation.

Abstract

After prompt and extensive débridement, defects of the pedion or the transitional region between it and the dorsum of the foot should be covered by an adequate tissue that can withstand statical stress. For reconstruction of the pedion the treatment of choice is the use of locally raised microsurgical island flaps with vascular pedicles. They can be stressed early and to a sufficient degree in well-vascularized tissue reconstruction. All the local flaps mentioned in the paper require good vascularization of the foot to provide a distributional region. In the case of large defects extending beyond the edge of the foot following severe trauma to the extremity microvascular free tissue transplantation is indicated.
