Zusammenfassung
Metatarsalia und Zehen sind wichtige Elemente des Fußgewölbes und statisch sowie dynamisch-funktionell extrem beansprucht. Nur eine korrekte anatomische und belastungsstabile Rekonstruktion sichert im Verletzungsfall die spätere volle Rehabilitation. I. und V. Strahl sind bei dislozierten und/oder offenen Frakturen direkt und rigide (Platten, Schrauben) zu stabilisieren. An den Strahlen II–IV stehen Schienungen mit Kirschner-Drähten im Vordergrund. Basisfrakturen am Os metatarsale V sind am besten mit Schrauben oder einer Drahtzuggurtung versorgt. Nichtdislozierte Frakturen können im Gipsschuh oder Unterschenkelgips erfolgreich behandelt werden. Dislozierte oder instabile Frakturen der Großzehe, besonders an den Gelenken, müssen operativ mit Miniplatten, ¶-schrauben oder Drähten behandelt werden. Die anderen Frakturen der Großzehe benötigen eine Ruhigstellung in Gipsschuh oder -longuette. Die Zehen II–V können in der Regel erfolgreich mit Tapes behandelt werden. Die jeweiligen Verletzungen erfordern in jedem Fall ein individualisiertes Therapiekonzept und -regime.
Abstract
The metatarsal bones and toes are important elements of the plantar arch and are subject to extreme statical and dynamic-functional stresses. Only an anatomically correct reconstruction that can remain stable under such stresses ensures complete rehabilitation following injury. In the case of dislocated and/or open fractures the Ist and Vth metatarsals must be stabilised by direct and rigid fixation with plates and screws. For metatarsal bones II–IV internal splinting with K-wires is mostly preferable. Fractures of the base of metatarsal V are best treated with screws or wire banding. Nondislocated fractures can be treated with a plaster shoe or boot. Dislocated or unstable fractures of the first toe, and especially of its joints, need operative treatment with miniplates and miniscrews or wires. Other types of fractures affecting the great toe require immobilisation with a plaster shoe or splint. Toes II–V can generally be successfully treated with tapes. The individual features of the injuries requires an individually tailored treatment concept and regimen in every case.
