Unaufgebohrter Tibiamarknagel (UTN)

Zusammenfassung

Die Resultate der Multizenterstudie bestätigen den unaufgebohrten Tibiamarknagel (UTN) als ein hervorragend geeignetes Implantat zur Behandlung von offenen und geschlossenen Tibiafrakturen mit kompromittierter periostaler Blutversorgung. Der Einsatz des UTN erfordert die sorgfältige Planung des Eingriffs anhand der Röntgenbildanalyse und unter Berücksichtigung des Weichteilschadens. Der im Vergleich zum Hohlnagel geringere Nageldurchmesser verringert den selbst zentrierenden Effekt des Nagels im Markrohr. Daher müssen (insbesondere für sehr distale Tibiafrakturen) sehr hohe Ansprüche an die Frakturreposition und die korrekte Wahl der Einschlagstelle gestellt werden. Distale Tibiafrakturen erfordern darüber hinaus obligat die 3-fache distale Verriegelung des Nagels. Zur Erhöhung der Stabilität der Versorgung sollte eine zusätzliche Fibulafraktur bei identischem Frakturniveau bei distaler oder mehrfragmentierter Tibiafraktur additiv stabilisiert werden.

Abstract

The results of this study confirm that the unreamed tibia nail (UTN) is an excellent implant for the treatment of open and closed fractures of the tibia when the periosteal blood supply is compromised. Application of the UTN requires critical analysis of the preoperative, intraoperative and postoperative radiographs and of the condition of the soft tissue. The small diameter of the nail reduces its self-centering effect, which means that fracture reduction and insertion of the nail must be extremely accurate, especially in the case of fractures in the very far distal region of the tibia. For such fractures, it is essential that three distal bolts should be used. To enhance stability, when there is a fibular fracture at the same level this should also be plated when the tibia is fractured in its distal fourth or a comminuted fracture of the tibia is present.
