Zusammenfassung
Posttraumatische Fußfehlstellungen sind häufig Folgen eines übersehenen Kompartmentsyndroms des Unterschenkels. Aber auch direkte Fußverletzungen führen zu Fehlstellungen. Nach Fersenbeinfrakturen ohne anatomische Rekonstruktion bildet sich ein Rückfußvalgus aus. Ziel der Therapie muss ein plantigrader Auftritt sein. Isolierte Verkürzungen und Kontrakturen der Achillessehne und der tiefen Beugemuskeln können durch ein mediales Weichteilrelease mit Verlängerung der Achillessehne und der des M. tibialis posterior verbessert werden. Die Valgusdeformität des Rückfußes wird durch additive Korrekturosteotomie ausgeglichen. Die Equinovarus-Adduktus-Fehlstellung erfordert eine Keilresektion im Bereich des subtalaren Gelenks mit lateraler Basis. Ein Adduktions-Supinations-Fehler in Höhe der Fußwurzel wird durch eine Keilresektion des Chopart-Gelenks beseitigt. Dies gilt auch ¶für die Spitzfußfehlstellung nach Kontraktur der plantaren Weichteile. Die Korrekturstellung wird durch eine Platten- oder isolierte Schraubenosteosynthese gesichert.
Abstract
Foot deformities often result from a compartment syndrome of the lower extremity that has gone unnoticed following trauma. The most common deformity is talipes equinovarus adductus. Valgus deformity of the hindfoot occurs if calcanear fractures are not reduced anatomically. In equinovarus deformities less than 20 degrees scar excision, dorsal capsular release in the ankle joint and lengthening of tendons in the deep lower leg compartment that have shortened following ischemic muscle necrosis are indicated. Closed wedge osteotomy at the level of the Chopart joint is needed for correction of the concomitant adduction failure of the midfoot. A laterally based closed-wedge osteotomy of the hindfoot is indicated if the varus deformity of the hindfoot is greater than 20 degrees. Necrosis of the plantar soft tissues results in equinovarus deformity with a maximum concavity at the level of the Chopart joint and is corrected by a dorsally based closed wedge osteotomy of this joint. In complex deformities these surgical techniques can be applied simultaneously. The valgus deformity of the hindfoot is corrected by an open wedge osteotomy with insertion of a tricortical inlay of the anterior iliac crest. After the osteotomy internal fixation is applied by plates or screws.
