Zusammenfassung
Für die gebohrte und ungebohrte Marknagelung von Ober- und Unterschenkelfrakturen können spezifisch gewichtete Vor- und Nachteile beider Verfahren aufgezeigt werden. Allgemein gültige Regeln zur Indikationsstellung lassen sich jedoch nicht aufstellen. Die Entscheidung „aufbohren, oder nicht?“ kann nur und muss patientenorientiert und individuell getroffen werden. Es gilt, unter Minimierung des Komplikationsrisikos für den Patienten das im Einzelfall günstigere Verfahren anzuwenden. Hierbei sind biologische und mechanische Aspekte unter Berücksichtigung der Frakturlokalisation (Femur, Tibia), des Frakturtyps, des Verletzungsmusters (Polytrauma, Lungenkontusion), der verfügbaren Implantate und der Operationstechnik von grundlegender ¶Bedeutung.
Abstract
Fixation for treatment of fractures of the tibia and femur can be performed with or without reaming of the shaft. Because there are no firm rules for deciding whether or not reaming is indicated, the specific advantages and disadvantages of each technique must be considered with reference to each patient’s specific problems to avoid complications. Besides biological and mechanical factors, type and localisation of the fracture and also any associated injuries (multiple injuries, pulmonary contusion), implants available and the surgeon’s level of experience are important factors that have to be considered.
