Zusammenfassung
Die Frage nach der optimalen postoperativen Pharmakotherapie nach peripheren arteriellen Rekonstruktionen und/oder Interventionen ist offen, obwohl sie ein ausgezeichnetes Modell darstellen, um verschiedene Antithrombotika zu testen. Dieser Artikel möchte einen Überblick darüber geben, was derzeit als gesichert gelten kann und versuchen, zukünftige Möglichkeiten der adjuvanten medikamentösen Behandlung aufzuzeigen.
Die Heparintherapie wird von vielen Gefäßchirurgen durchgeführt, allerdings kann für ihren Einsatz keine verbindliche Empfehlung abgegeben werden.
Zu den Medikamenten, die die Thrombozytenfunktion stabilisieren, gehört Acetylsalicylsäure (ASS). ASS in mittlerer Dosierung ist die am intensivsten geprüfte Substanz, sodass ihre langzeitliche (evtl. lebenslange) Verwendung sowohl hinsichtlich der Verbesserung der Offenheit als auch hinsichtlich der sekundären Prävention empfohlen werden kann. Ticlopidine dürfte viel versprechende Potenzen haben, eine entsprechende Bestätigung steht aber noch aus. Ähnliches dürfte für Clopidogrel gelten.
Orale Antikoagulanzien werden empfohlen, allerdings sind eine Reihe von klinischen Prüfungen, mit zum Teil uneinheitlichen Ergebnissen vorhanden, sodass man eine Empfehlung für ihre Rezeptur nur mit Vorsicht aussprechen kann. Eine direkte multizentrische randomisierte Vergleichsuntersuchung Aggregationshemmer vs. orale Antikoagulanzien, die eine differenzierte Politik empfiehlt, wurde kürzlich veröffentlicht. Patienten mit autologen Venenumleitungen werden besser mit oralen Antikoagulanzien, solche mit alloplastischen Rekonstruktionen mit Aggregationshemmern geschützt.
Die Kombination von Aggregationshemmern und oralen Antikoagulanzien wäre eine Option, ist allerdings durch Risiken belastet. Kombinationstherapien in verringerter Dosis könnten eine mögliche Alternative darstellen.
Über niedrig molekulares Dextran liegt eine einzige klinische Prüfung hinsichtlich einer kurz dauernden Funktionsverbesserung vor.
Abstract
The question of how to best employ postoperative pharmacotherapy following peripheral artery reconstructions and/or interventions remains controversial, although peripheral vascular reconstructions provide an excellent model for testing various antithrombotic regimens. The overview is intended to evaluate current attitudes and discusses future aspects of adjunctive treatment.
Heparin is widely used among vascular surgeons, and although the methods of administration vary, only a general recommendation may be given.
Platelet active agents, acetylsalicylic acid (ASA, aspirin) in particular, in medium dosage, are the most intensely tested substances, so the recommendation for its long-term (lifelong?) use regarding patency, survival and prevention of vascular events in general is based on a high level of evidence. Many large trials and metaanalyses have been conducted. Ticlopidine is promising, but requires further corroborating evidence. Similarly Clopidogrel seems to be at least equally effective and an alternative to ASA.
The recommendation of oral anticoagulants seems to be based upon several single centre trials with controversial results, so a word of caution is justified. Recently, a direct multicentre randomised comparison of oral anticoagulants versus antiaggregants has been published recommending a differentiated policy, i.e. prescribing anticoagulants to patients with vein grafts and antiaggregants to patients with alloplastic grafts.
The combination of antiplatelet drugs and anticoagulants might be a worthwhile option, although the risk of bleeding complications is substantial. The combination of the two, but in reduced dosages, might turn out to be attractive.
Low molecular weight dextran has been shown in a multicentre trial to be effective for short term use.
