Zusammenfassung
Die Angioplastietechnik stellt einen wesentlichen Bestandteil in der Behandlung des chronischen Verschlusses der A. femoralis superficialis bei symptomatischen Patienten dar. Die endovaskuläre Rekanalisierung auch längerstreckiger Verschlüsse hat sich durch Weiterentwicklung des Instrumentariums zu einem sicheren und komplikationsarmen Verfahren entwickelt. Die Langzeitergebnisse konnten allerdings in den letzten 20 Jahren nicht entscheidend verbessert werden. Die Reobstruktionsrate liegt bei etwa 20–50% nach 2–3 Jahren, auch bei Verwendung von endoluminalen Stents.
Kriterien zur indikatorischen Stratifizierung der Patienten sind das klinische Stadium (SCVIR-Kategorien), eine gute Runoff-Situation der Unterschenkelgefäße und die Bereitschaft zur Risikofaktorreduktion. Das hämodynamisch gute Primärergebnis, die rechtzeitige Terminierung von Reinterventionen und die medikamentöse Nachbehandlung sind wichtige Aspekte zur Verbesserung der Langzeitoffenheitsrate.
Abstract
Infrainguinal bypass procedures and percutaneous transluminal angioplasties (PTA) have been used to treat patients with symptomatic obstructions of the femoropopliteal artery. PTA is often the primary procedure to avoid the higher rate of morbidity associated with operation. However, long-term durability of balloon dilatation or endovascular stenting have been questioned, despite favorable primary success rates for endovascular recanalization (>90%).
Clinical presentation (claudication or critical limb ischemia), quality of primary PTA result and quality of distal runoff, risk factors, comorbidity, and medication are contributing factors to long-term success. After endovascular recanalization, 2- to 3-year patency rates of 50–70% can be achieved. Additional stent implantation is helpful for removing immediate post-PTA residual stenoses. The wide range of clinical results reported in the literature can be attributed mainly to the method of patient selection.
