Zusammenfassung
Das schwierigste an der lumbalen Sympathektomie war und ist die richtige Indikation, die über Nutzen und Schaden der Methode entscheidet. Deshalb kann und darf die CT-gesteuerte Sympathikolyse als weniger invasive neue Methode sich keine Zusatzindikationen schaffen. In Anbetracht der peripher rekonstruktiven Möglichkeiten beschränkt die lumbale Sympathektomie sich heute auf:
additive Maßnahmen zur arteriellen Rekonstruktion zur gezielten Beeinflussung vorhandener Kollateralen im Oberschenkel- und Unterschenkelbereich sowie die Senkung des peripheren Strömungswiderstandes im kruralen/pedalen Bereich,
die isolierte lumbale Sympathektomie als Ultima Ratio im Stadium III und frühem Stadium IV bei Fehlen einer peripheren Anschlussmöglichkeit und
die marginal bedeutsame Beeinflussung der Nozizeption.
Der Nutzen einer additiv durchgeführten LS (etwa nach iliakoprofundaler Revaskularisation) scheint erwiesen. Demgegenüber ist der Effekt einer alleinigen Sympathektomie als Ultima Ratio im Einzelfall nicht prognostizierbar und bedarf theoretisch einer – wenn auch aufwendigen – Testung, wenn Fehlschläge vermieden werden sollen. Als Faustregel kann allenfalls gelten, dass unterhalb eines Dopplerdrucks von 40 mmHg und eines ptcO2 von 20 mmHg in der peripheren Perfusionsgrenzzone die Wahrscheinlichkeit eines therapeutischen Fehlschlages sehr hoch einzuschätzen ist.
Abstract
The greatest difficulty in deciding to carry out a lumbar sympathectomy is to find adequate indications predicting the success or failure of the procedure. Therefore the less invasive CT-guided technique is not capable of providing its own indications. On account of recent peripheral reconstructive possibilities, the application of lumbar sympathectomy today is restricted to three situations:
as an adjunctive measure following iliaco/profundal reconstructive inflow operations to the collateral vessels of the thigh,
as a last attempt to improve crural and pedal microcirculation in the absence of any possibility of peripheral reconstruction in stage III and early IV,
pain relief by interference with spinal pain transmission.
As an adjunct to arterial reconstruction the favorable effect of LS seems to be established. In contrast, the effectivness of an ultimate choice LS cannot be foreseen for the individual case and therefore – theoretically – has to be subjected to an admittedly complicated test if an unsuccessful outcome is to be avoided. As a rule of thumb, a distal perfusion pressure limit of 40 mmHg and a pedal ptcO2of 20 mmHg may be judged as marginal limits for a beneficial effect.
