ASS und Heparine:
Grundlage der antithrombotischen Therapie bei instabiler Angina pectoris ist nach wie vor die Acetylsalicylsäure (ASS), deren zuverlässige Wirksamkeit in zahlreichen Studien gezeigt werden konnte. Dennoch muss die Frage einer optimalen Dosierung weiterhin offen bleiben. Die alleinige Gabe von Heparinen zeigt bei der instabilen Angina pctoris nur geringe Wirksamkeit. Dagegen ist die gleichzeitige Gabe von ASS + Heparin wirksamer als die alleinige Gabe von ASS, sodass diese Kombination die Grundlage einer antithrombotischen Therapie bei instabiler Angina pectoris darstellt.
Ticlopidin und Clopidogrel:Über Thienopyridine (Ticlopidin und Clopidogrel) liegen bisher nur wenige Daten in der konsevativen Therapie der instabilen Angina pectoris vor, wobei sich Ticlopidin wirksamer erwiesen hat als Plazebo. Vergleichende Studien über die Wirksamkeit von Clopidogrel + ASS versus ASS alleine sollen in Kürze veröffentlicht werden.
Therapieempfehlung nach Koronarintervention: Nach koronarer Intervention (Stents) hat sich die Kombination von ASS + einem Thienopyridin als die optimale Therapie zur Verhinderung einer Stentokklusion erwiesen. Diese überlegene Wirksamkeit konnte sowohl für Ticlopidin als auch für Clopidocral gezeigt werden. Da aufgrund der bisher vorliegenden Daten davon ausgegangen werden kann, dass das Sicherheitsprofil von Clopidogrel dem von Ticlopidin überlegen ist, wird man Clopidogrel (1-mal täglich 75 mg oral) den Vorzug geben. Aufgrund der bisherigen Studienlage sollte die Kombination ASS + Clopidogrel mindestens 4 Wochen nach Setzen eines intrakoronaren Stents gegeben werden.
Aspirin and Heparin:
In several studies aspirin has been found to be very effective in unstable angina pectoris reducing fatal and non-fatal myocardial infarction by 50–70%. Unfortunately the optimal dose of aspirin is still an open question. Whereas heparin alone shows only a weak effectiveness the combination of aspirin and heparin is superior to aspirin alone and is still the basis of antithrombotic therapy in unstable angina.
Ticlopidine and Clopidogrel: Experience with thienopyridine derivatives in unstable angina is limited. Ticlopidine has been found to be superior to aspirin alone. Data with the combination of clopidogrel and aspirin should be available soon.
Therapeutic Recommendation after Coronary intervention: Both, ticlopidine and clopidogrel have been found to be very effective in preventing coronary-stent thrombosis when combined with aspirin. Meanwhile ticlopidie has been widely substituted by clopidogrel due to the better safety profile of the latter one. 75 mg clopidogrel daily combined with aspirin is recommended for at least 4 weeks after coronary stenting.
