Am häufigsten ergibt sich die Indikation für spätrekonstruktive Eingriffe an der Halswirbelsäule bei chronischen Instabilitäten, veralteten, primär übersehenen Luxationen sowie sekundär auftretenden progredienten neurologischen Symptomen. Die Reposition erfordert meist das kombiniert dorsoventrale Vorgehen, bei neurologischen Symptomen sind nicht selten zusätzliche dekomprimierende Maßnahmen notwendig. Eine häufige Ursache für Spätversorgungen an der oberen Halswirbelsäule besteht bei Denspseudarthrosen nach Frakturen vom Typ Anderson II, hier wird der atlanto-axialen Fusion der Vorzug gegeben.
Indications for late reconstructive surgery at the cervical spine result from chronic instability, old displacements that had originally been overlooked, or delayed progressive neurological malfunction. In most cases, the reduction demands a combined dorsoventral procedure including posterior release and anterior fusion. In the presence of neurological deficits, additional decompression techniques are recommended. Another reason for late reconstruction at the upper cervical spine is given in the presence of dens pseudarthrosis following fractures of Anderson type II, for which atlanto-axial fusion is the procedure of choice.
