Preaortic Iliac Confluence – eine seltene venöse Lageanomalie

Zusammenfassung

Wir berichten über einen 73-jährigen Patienten, bei dem sich im Rahmen einer elektiven Aneurysmaresektion der Aorta abdominalis eine seltene Lageanomalie der V. cava inferior fand mit Zusammenfluss der Vv. ilacae zur V. cava ventral der Aortenbifurkation (“preaortic ilac confluence”). Die Rekonstruktion der infrarenalen Aorta erfolgte durch Interposition eines aortobiiliakalen Y-Bypass in anatomischer Lage ventral der venösen Gefäße. Der postoperative Verlauf war komplikationslos. Wichtig ist die Kenntnis dieser (seltenen) Verlaufsvariante. Im Rahmen der präoperativen radiologischen Diagnostik sollte gezielt nach venösen Lageanomalien gesucht werden.

Abstract

We report on a 73-year-old man undergoing elective resection of an aneurysm of the abdominal aorta, in whom a very rare position anomaly of the inferior vena cava, namely, union of the iliac veins with the vena cava ventral to the aortic bifurcation (preaortic iliac confluence), was found. The infrarenal aorta was reconstructed by interposing an aorto-biiliacal Y bypass in an anatomical position ventral to the venous vessels. The postoperative course was complication free. It is important to be aware of the possibility of this – admittedly rare – anomaly. During the preoperative radiological diagnostic exploration, a specific search should be made for possible venous position anomalies.
