Management of vascular and lymphovenous malformations

Abstract

Perhaps the most important event in relation to the vascular and lymphovenous group of malformations has been the development of a classification which is useful to physicians, patients, and parents. The broad classification consists of hemangiomas and vascular malformations; the latter group is further subdivided into venous, arteriovenous, and lymphovenous malformations [5, 11]. Hemangiomas are present at or just after birth in 40% of infants. They grow rapidly and involute slowly; there is increased endothelial cell activity. Venous malformations are present at birth in 70% of cases; they grow with the child and have a normal rate of endothelial cell turnover. Lymphovenous malformations occur at an early age and may or may not resolve. Arteriovenous malformations are usually progressive. The treatment is a combination of embolization and surgery.

Im Zusammenhang mit der Gruppe vaskulärer und lymphovenöser Mißbildungen war die Entwicklung einer für Ärzte, Patienten und Eltern nützlichen Klassifizierung in letzter Zeit vielleicht der wichtigste Fortschritt. Die grobe Klassifikation beinhaltet Hämangiome und vaskuläre Mißbildungen; die Gruppe der letzteren ist in venöse, arteriovenöse und lymphovenöse unterteilt [5, 11]. Hämangiome finden sich bei oder gleich nach der Geburt bei 40% der Kinder. Sie wachsen rasch und bilden sich nur langsam zurück; bei ihnen besteht eine erhöhte Aktivität der Endothelzellen. Venöse Mißbildungen finden sich bei der Geburt in 70% der Fälle; sie wachsen mit dem Kind und weisen eine normale Rate der Endothelzellenerneuerung auf. Lymphovenöse Mißbildungen finden sich im frühen Alter; ihre Rückbildung ist möglich. Arteriovenöse Mißbildungen sind normalerweise progressiv. Die Behandlung besteht in einer Kombination von Embolisation und Operation.
