Zusammenfassung
Während die mündungsnahe Ligatur der V. saphena magna bei der Behandlung der V.-saphena-magna-Insuffizienz sich als Standardmethode etablierte, so finden sich bei der Abklärung der Rezidivvarikosis im Kniekehlen-bereich als Ursache zu lange V.-saphena-parva-Stümpfe und zwar in unserem Patientengut in einer Häufigkeit, die darauf schließen läßt, daß die Absetzung der V. saphena parva am Niveau der V. poplitea nicht generell durchgeführt wird, obgleich ähnliche hämodynamische Gegebenheiten vorliegen wie beim saphenofemoralen Reflux. Im Zeitraum von 1990–1996 waren, bei primär auswärts operierten Patienten, die Rezidiveingriffe in der Kniekehle im Verhältnis häufiger, als in der Leiste und in der Regel fand sich ein zu lang belassener V.-saphena-parva-Stumpf. Im eigenen Krankengut hingegen lag der Reflux nach V.-saphena-parva-Operation bei 6,4%, nach einer Beobachtungszeit von 2 Jahren, was wir darauf zurückführen, daß die V. saphena parva am Niveau abgetragen wurde.
Abstract
The ligation of the greater saphenous vein at the saphenofemoral junction is a common procedure to treat such a vascular insufficiency. The control of recurrent varicosis in the popliteal region showed that there is a close relationship to small saphenous vein stumps which are too long. This indicates that ligation of the small saphenous vein at the popliteal junction is not often done although the haemodynamics are similar to those in the saphenofemoral reflux. In patients treated from 1990 to 1996 we could see that in those treated elsewhere the reoperation rate in the popliteal area was higher than in the groin region and that in most cases we found that the small saphenous vein stump was too long. In our cases we have seen that after primary operation in the popliteal area the reflux from the small saphenous vein stump was only in 6.4% after 2 years, which shows that correct ligation of the small saphenous vein helps to reduce the recurrence of varicosis in the popliteal area.
