Zusammenfassung
Trotz einer langen Begriffsgeschichte existieren immer noch Erkenntnislücken im Verständnis des Konversionsvorgangs. Nach wie vor erscheint der „Sprung aus dem Seelischen in die somatische Innervation” (Freud 1909a) geheimnisvoll. Zwar wurde der erste Schritt des Konversionsvorgangs als Verdichtung einer konflikthaften Vorstellung auf eine mit dieser assoziierten Körperrepräsentanz bereits von Freud (1915) zutreffend beschrieben, die Umwandlung dieser verdichteten Vorstellung in reale Wahrnehmung bzw. motorische Aktionen ist bis heute aber nicht wirklich begriffen. Der Autor versucht, ausgehend vom Freudschen Konversionsbegriff und unter Berücksichtigung neuerer Konzepte diesen zweiten Schritt im Konversionsprozeß nachzuzeichnen. Dabei läßt sich zeigen, daß der „Sprung” durch ein komplexes Zusammenspiel regressiver und progressiver Prozesse, durch eine spezifische Kombination von Primär- und Sekundärvorgang, möglich wird. Zum Schluß der Arbeit wird der Konversionsvorgang an einer Krankengeschichte exemplarisch dargestellt.
Summary
Although the concept of conversion has a long history, the mechanism of conversion is still not adequately understood. The „leap from the psychic to the somatic innervation” (Freud 1919a) remains as mysterious as it ever was. Freud’s (1915) presumption that the first stage in conversion is the condensation of a conflictuous idea with a psychic representation of a part of the body associated with the idea is undoubtedly correct. What is not fully understood is how such a condensed idea can be transformed into actual perception or into motions of the body. The author examines the second stage of conversion mechanism in the light of Freud’s and also more recent conceptions. It can be shown that the „leap” is the result of an intricate interplay between regressive and progressive processes, i.e. of specific combinations of the primary and the secondary process. Finally the author exemplifies the conversion mechanism with a short case history.
