Polydactyly of the foot

Summary

Polydactyly may be preaxial or tibial (halluxside), postaxial or ulnar (side of the little toe) and central (middle toes). The duplication may appear at the distal and medial phalanges or at the whole digit. The metatarsal bone may be partwise or completely duplicated, the accessory toes may share only one metatarsal. Surgical intervention may be indicated in shoeproblems, for esthetic reasons or, especially in duplication of the metatarsales, because of secondary deviation of the toes and therefore shoeproblems or plantar callosities. Preoperative analysis including x-ray is of great importance to achieve good functional and cosmetic results.

Zusammenfassung

Die Polydaktylie am Fuß– nach Definition überzählige Zehe, unabhängig vom Niveau der Verdoppelung, der Art der Verdoppelung und der Funktion der akzessorischen Zehe – kann in eine präaxiale oder tibiale (Großzehenstrahl), postaxiale oder fibulare (Kleinzehenstrahl) und zentrale (2. bis 4. Strahl) Vermehrung eingeteilt werden. Die Duplikation kann die proximale, mittlere und distale Phalanx oder die ganze Zehe betreffen. Das Metatarsale kann teilweise oder ganz verdoppelt sein, die akzessorischen Zehen können sich artikulär einen gemeinsamen Mittelfußknochen teilen. Eine operative Therapie kann aus ästhetischen Gründen oder, vor allem bei Duplikationen der Metatarsalia – die großteils mit einer Formveränderung des Fußes und Verdrängung der Zehen einhergehen und damit Belastungs- und Platzprobleme im Schuh verursachen – indiziert sein. Der präoperativen Klassifikation anhand Klinik und Röntgen kommt große Bedeutung zu, um funktionell und kosmetisch korrekte Ergebnisse zu erreichen.
