Analysis of the acetabular migration profile in rheumatoid arthritis compared with cementless acetabular revision

Summary

In a retrospective analysis the migration profile of 73 cementless total hip implants (Zweymüller Alloclassic) in 54 patients with rheumatoid arthritis were compared with the results of 58 implants of the same design in 50 patients undergoing cementless revision after loosening of an Endler cup. 258 conventional radiographs of the rheumatoid patients and 164 radiographs of the revision group were digitized. The migration analysis was performed using a modified Dickob's technique that allows for correction of different X-ray magnifications as well as horizontal and vertical tilting of the pelvis. The analysis of migration exceeded 2 mm in eight rheumatoid patients. In three of them clinical loosening of the acetabular cup was obvious. In the group of patients after cup revision a significantly higher amount of cranial migration was detected compared with the rheumatoid patients. This phenomenon could be explained by the destruction of the weight-bearing cranial acetabular sclerosis after reaming.

Zusammenfassung

Rheumatoide Arthritis (RA) und Wechseloperationen stellen in der Hüftendoprothetik wegen der entweder primär verminderten Qualität des periacetabulären Knochenlagers oder sekundärer Destruktion eine besondere Herausforderung dar. In einer retrospektiven Analyse wurde das Migrationsverhalten einer zementfreien konischen Schraubpfanne (Zweymüller-Alloclassic) bei 54 Patienten mit 73 Hüfttotalendoprothesen mit RA und bei 50 Patienten, bei denen 58mal ein zementfreier Pfannenwechsel durchgeführt wurde, verglichen. 258 Standardhüftvergleichsröntgen der Rheumapatienten und 164 Hüftvergleichsröntgen der Gruppe nach Wechseloperationen wurden mit Hilfe digitaler Bildanalyse nach einer von Dickob et al. angegebenen, modifizierten Methode analysiert. Dabei werden die Röntgenbilder hinsichtlich unterschiedlicher Vergrößerungsmaßstäbe kallibriert, und eine mögliche horizontale oder vertikale Beckenverkippung bei Kontrollaufnahmen korrigiert. 8 Patienten mit rheumatoider Arthritis wiesen Migrationen über 2 mm auf, bei 3 von ihnen war eine Pfannenlockerung klinisch manifest. Die Gruppe der Patienten nach Pfannenrevision zeigte gegenüber der Patientengruppe mit RA eine signifikante Migrationstendenz nach kranial. Dies wird durch den Verlust von lasttragender Sklerose im Bereich des kranialen Pfannenanteils erklärt.
