Systematische Therapie des Zervixkarzinoms

Zusammenfassung

Bis Mitte der 70er Jahre spielte die Chemotherapie im gesamten Behandlungskonzept nur eine marginale Rolle. Auch heute sind Operation und Strahlentherapie unverändert die Eckpfeiler der Behandlung. Für den Einsatz der Chemotherapie gibt es prinzipiell folgende Indikationsbereiche:

Neoadjuvante Chemotherapie des fortgeschrittenen Primärkarzinoms (FIGO-Stadium IIb, III und IV), entweder vor der vorgesehenen Operation oder Strahlentherapie

Simultane Radio-Chemo-Therapie

Postoperative adjuvante Chemotherapie

Palliative Chemotherapie (Lokalrezidive, Fernmetastasen)

Während die palliative Chemotherapie als etabliert bezeichnet werden darf, stellen die 3 erstgenannten Indikationen keine Standardtherapieempfehlung dar und müssen als experimentell bezeichnet werden. Diese Einschränkung ist hauptsächlich auf das Fehlen verbindlicher Daten aus prospektiv randomisierten Studien zurückzuführen und muß bei der Aufklärung der Patientin Berücksichtigung finden.

Summary

Up to the middle of the 1970s, chemotherapy had only a marginal role in the overall treatment concept for cervical carcinoma, and surgery and radiotherapy are still the cornerstones of treatment today. There are indications for chemotherapy, which can theoretically be used as follows:

As a neoadjuvant therapy for an advanced primary carcinoma (FIGO stages IIb, III and IV), before the planned operation or before radiotherapy

At the same time as radiotherapy (chemoradiotherapy)

As postoperative adjuvant therapy

For palliation (local recurrences, distant metastases)

Whilst palliation can be regarded as an established indication for chemotherapy, the first three uses cannot be regarded as standard treatment recommendations and must be described as experimental. This reservation is attributable mainly to the lack of confirmatory data recorded in prospective randomized studies and must be discussed when the patient is given information on her condition and the treatment options.
