Spontaneous splenic rupture with diffuse angiomatosis of the spleen

Summary.

The diffuse angiomatosis of the spleen is a rare splenic proliferation. Spontaneous rupture is the main, potentially lethal complication, found in 25 % of cases. The problem with these vascular tumors is preoperative diagnosis, because they are asymptomatic for a long time. First-choice therapy is limited resection. In the case of rupture, splenectomy is a life-saving procedure. We report on a 56-year-old woman with a spontaneous splenic rupture as the first symptom of diffuse angiomatosis of the spleen. Exceptional features of splenic hemangiomas, causes of spontaneous rupture and the therapy are described.

Zusammenfassung.

Die diffuse Angiomatose der Milz ist eine seltene lienale Neoplasie. Die spontane Ruptur stellt die wichtigste, potentiell letale Komplikation dar und tritt in ca. 25 % der Fälle auf. Das Problem besteht in der Schwierigkeit der präoperativen Diagnostik, da die vasculären Tumoren lange Zeit symptomlos bleiben. Als Therapie ist eine organerhaltende Tumorentfernung anzustreben, während bei der Ruptur die Splenektomie oft lebensrettend ist. Wir berichten über eine 56 jährige Patientin mit spontaner Milzruptur als Erstsymptom der diffusen Angiomatose. Dargestellt werden die Besonderheiten der lienalen Hämangiome, die Ursachen der spontanen Milzruptur und die Therapiemaßnahmen.
