Zusammenfassung
Sevofluran zeichnet sich insbesondere durch einen sehr niedrigen Blut/Gas-Verteilungskoeffizienten von 0,69 aus, der nur durch Desfluran und Lachgas unterschritten wird. Aus diesem Grunde findet eine rasche alveoläre Äquilibrierung statt, die zur raschen Narkoseeinleitung genutzt werden kann. Wegen seines angenehmen Geruchs ist eine Narkoseeinleitung per inhalationem möglich und auch klinisch praktikabel. Eine Nephrotoxizität der durch Metabolismus entstehenden Fluoridionen und des durch Degradation am Atemkalk entstehenden Compound A beim Menschen konnte bisher nicht nachgewiesen werden. Die pulmonale Elimination des Sevofluran geschieht wegen der niedrigen Blut-Löslichkeit ebenfalls rasch. Eigene Untersuchungen konnten zeigen, daß das Erwachen nach Sevoflurannarkosen ebenso rasch erfolgt, wie nach Desflurannarkosen. Die chemisch-physikalischen Eigenschaften des Sevofluran erlauben seine Anwendung in konventionellen Vaporen.
Abstract
Sevoflurane is characterized by a low blood/ gas partition coefficient of 0.69, only desflurane and nitrous oxide have lower blood/gas solubilities. Alveolar equilibration is fast, a feature useful for rapid induction of anesthesia. Because of its pleasant smell, mask induction is feasible and routinely used in clinical settings. Formation of inorganic fluoride by metabolism and by compound A by degradation in CO2-absorbent has not yet been shown to be nephrotoxic in humans. Pulmonal elimination of sevoflurane is rapid because of its low blood solubility. Clinical results showed that rapidity of recovery from sevoflurane anesthesia is equal to that of desflurane anesthesia. Physicochemical properties of sevoflurane allow its application in conventional vaporizers.
