Operationsziel
Zügelungsoperation der Patella zur Behandlung einer rezidivierenden Patellaluxation oder -subluxation durch laterale Retinakulumspaltung und Ansatzverlagerung des Musculus vastus medialis nach distal und lateral auf die Kniescheibe.
Indikationen
Posttraumatisch rezidvierende Patellaluxation oder -subluxation nach lateral nach erfolglosem Muskelaufbautraining.
Habituelle Subluxation oder Luxation der Patella nach lateral.
Kontraindikationen
Kongenitale Patellaluxation.
Degenerative Veränderungen des gesamten Patellofemoralgelenks.
Genu valgum >10° (Tuberositasverletzung oder Korrektur der Beinachse erforderlich).
Ausgeprägte Schwäche des Musculus vastus medialis, z. B. im Rahmen einer neurologischen Erkrankung.
Operationstechnik
Durchtrennung der zur Patella einstrahlenden Fasern des Tractus iliotibialis sowie des Retinaculum extensorum laterale (sog. Retinakulumspaltung nach Viernstein) über einen lateralen, parapatellaren Hautschnitt.
Versetzung des Ansatzes des Musculus vastus medialis um 1–2 cm nach distal und lateral auf die Patella.
Ergebnisse
Alle 21 Patienten (16 weiblich, fünf männlich), die sich zwischen 1989 und 1993 einer proximalen Zügelungsoperation der Patella nach Insall unterzogen hatten, konnten durchschnittlich 6,3 Jahre (4 Jahre und 5 Monate bis maximal 8 Jahre und 3 Monate) nach dem Eingriff nachuntersucht werden. 13–mal wurde die Diagnose einer habituellen Patellaluxation gestellt; bei acht Patienten war ein adäquates Primärtrauma berichtet worden. Eine allgemeine Bandschwäche wurde bei drei Patienten festgestellt. Bei einem Patienten trat postoperativ eine Wundinfektion auf. Zwei Patienten beurteilten das Ergebnis subjektiv als exzellent, 13 Patienten als gut, sechs Patienten als ausreichend und kein Patient als schlecht. Bei der objektiven Beurteilung wurden sieben Patienten mit sehr gut, neun mit gut und fünf mit ausreichend bewertet. Bei drei Patienten kam es zu einem erneuten Auftreten (ein- bis dreimal) der Patellaluxation oder -subluxation. Bei einem Patienten musste wegen rezidivierender Luxationen eine operative Revision durchgeführt werden. Bei den beiden anderen Patienten traten nach einer gezielten Muskelkräftigung bisher keine weiteren Instabilitätssymptome auf.
Abstract
Objective
Proximal realignment of the patella for the treatment of patellar subluxation or dislocation consisting of a lateral release and advancement of the vastus medialis.
Indications
Recurrent lateral subluxation or dislocation of the patella despite a supervised exercise program.
Recurrent subluxation or dislocation of the patella.
Contraindications
Congenital dislocation of the patella.
Generalized degenerative arthritis of the patellofemoral joint.
Excessive valgus deformity of the knee.
Moderate to severe muscular atrophy of the vastus medialis muscle (i.e., in neurologic disorders).
Surgical Technique
Lateral parapatellar incision of skin. Detachment of the fibers of the iliotibial tract and the lateral retinaculum from the lateral patella. Medial capsular incision extending from the quadriceps tendon over the patella into the patellar ligament. The quadriceps expansion is shaved from the medial third of the patella preserving the longitudinal continuity. The vastus medialis is advanced and sutured onto the middle and lateral aspects of the patella.
Results
Out of 21 patients (16 women, five men) who received a proximal realignment procedure during 1989 and 1993, all could be evaluated after 6.3 years (minimum follow-up of 4 years 5 months, maximum of 8 years 3 months). In 13 patients the diagnosis was recurrent dislocation of the patella, in eight patients a true primary traumatic dislocation was noted. A generalized ligamentous laxity was seen in three patients. One patient had a superficial wound infection postoperatively. Two patients had an excellent, 13 patients a good, six patients a fair and no patient a poor result according to the subjective score of Turba et al. Three patients experienced a recurrence of instability (one to three events) after the operation. One of these patients had to be revised for postoperative recurrent dislocation of the patella. The other two patients experienced no more symptoms of instability after muscle strengthening exercises of the vastus medialis muscle.
