Zusammenfassung
Hintergrund: Inhalative Steroide gelten seit langem als Basistherapie des Asthma bronchiale. Sie inhibieren die Transkription proinflammatorischer Zytokine und blockieren dadurch die humorale und zelluläre Entzündung beim Asthma. Von β2-Sympathikomimetika (β2-Agonisten) ist seit langem bekannt, dass sie neben ihren bronchodilatativen Eigenschaften auch entzündungsmodulierende Fähigkeiten besitzen.
Synergismus: Neue Arbeiten zeigen, dass β2-Agonisten über die Aktivierung des β2-Rezeptors zu einer glucocorticosteroidunabhängigen Aktivierung des Glucocorticoidrezeptors führen. Sie erhöhen damit die Konzentration des Glucocorticosteroidrezeptors in der Zelle und führen zu einer verbesserten Wirkung von Glucocorticosteroiden. Diese Synergie von β2-Agonisten und Steroiden auf molekularer Ebene erklärt den klinisch zu beobachtenden überadditiven Effekt der Kombination dieser beiden Substanzen.
Abstract
Background: Due to the potent anti-inflammatory strength, glucocorticosteroids are the basis of long-term (controller) therapy in asthma. They inhibit transcription of pro-inflammatory cytokines and block humoral as well as cellular derived inflammation in the bronchi. β2 sympathomimetics are well known to cause dilatation of the bronchi but also to modulate certain anti-inflammatory responses.
Synergism: Just recently it could be demonstrated that β2 agonists may also be able to activate glucocortico osteroid receptors, independently of existing glucocortiosteroids. Thus, β2 agonists lead to augmentation of glucocorticosteroid receptor concentration, and consequently cause better efficacy of glucocorticosteroids within the cell. Vice versa glucocorticosteroids enhance the transcription rate of β2 receptors, causing higher β2 receptor number, and thus may contribute to better β2 agonist efficacy. These synergies of β2 agonists and glucocorticosteroids on molecular level explain additive efficiency in the therapy of asthma – particularly through improving lung function – when using both substances in combination.
