Abstract
Carpal tunnel release is frequently followed by weak grasp but recently, endoscopy has been claimed to avoid this pitfall. 277 patients representing 303 CT were enrolled in a prospective randomized study to assess changes in grasp after classical open CTR (Group I - 77 cases), ligamentoplasty (Group II - 133 cases) and an Agee endoscopic CTR (Group III - 99 cases). Grasp was measured pre-operatively and post-operatively every month for six months to analyse strength recovery according to technique, dominance, sex, motor conduction velocity, job, and leisure activity.
There were no complications in the series except one RSD and six open conversions in the Agee group due to technical problems.
Pre-operative CT was accompanied by an average loss of strength of 18%. In the postoperative period, the contralateral side had an increase in strength from 2% to 12.5%. These data plus the frequent bilaterality of CT explained the bias of any post-operative study without pre-operative measurement.
Strength recovered faster and better in Group II and III. At three months, the percentage of patients having recovered was 38% in Group I, 54% in Group II, 58% in Group III. An explanation of similarity in the last two groups could be found in the modification of Agee procedure preserving an intermediate fascia, avoiding full separation of the two edges of the ligament, as in the ligamentoplasty.

La libération du canal carpien est fréquemment suivie d’une perte de la force de serrage mais récemment la chirurgie endoscopique a été prônée comme pouvant éviter cet inconvénient. 277 patients représentant 303 canaux carpiens (GC) ont été inclus dans une étude randomisée afin de pouvoir évaluer la force de serrage après libération chirurgicale classique du CC (Groupe I - 77 cas), après ligamentoplastie (Groupe II - 133 cas) et après libération endoscopique du CC selon Agee (Groupe III - 99 cas). La force de serrage a été mesurée avant l’intervention puis en postopératoire pendant six mois, à raison d’une fois par mois afin d’analyser la récupération de la force selon la technique, le côté dominant, le sexe, la vitesse de conduction motrice, le travail et les activités de loisir.
Il n’y a pas eu de complications dans les séries, excépté une algodystrophie et la nécessité de pratiquer six ouvertures classiques pendant la chirurgie endoscopique d’Agee, pour des problèmes techniques.
L’état préopératoire des CC correspondait à une perte de force dans 18 % des cas. Dans la phase postopératoire, l’on consate une augmentation de la force qui va de 2 à 12,5 %. Ces faits ajoutés à la fréquente bilatéralité de l’atteinte, explique le biais de toute étude qui ne prend pas en compte une évaluation préopératoire de la force de serrage.
La force récupère plus vite et mieux dans les Groupes II et III. À trois mois, le pourcentage de patients qui ont récupéré était de 38 % dans le Groupe I, de 54 % dans le Groupe II et de 58 % dans le Groupe III.
Une explication à la similitude des résultats dans les deux derniers groupes peut être donnée par la modification de la technique d’Agee par conservation d’un fascia intermédiaire, évitant la séparation complète des deux lèvres du ligament, comme dans la ligamentoplastie.
