Zusammenfassung
Neben der zunehmenden Verbesserung der Dokumentation der klinischen Wirksamkeit der subkutanen spezifischen Immuntherapie (SCIT) und in den letzten Jahren auch der sublingualen spezifischen Immuntherapie (SLIT) stehen immer mehr Daten zu deren immunologischen Wirkmechanismen zur Verfügung, die zu einem deutlich besseren Verständnis dieser Behandlungsformen einschließlich ihrer differenziellen Indikationsstellung und besonderen klinischen Effekte (Nachhaltigkeit, präventive Wirkung) beigetragen haben. Das bevorzugte Erklärungsmodell bezüglich der immunologischen Wirkmechanismen stellt die Verschiebung des Gleichgewichts von allergenspezifischen TH2-Zellen hin zu TH1-Zellen und die Induktion bzw. Aktivierung oder Rekrutierung von IL-10- und „transforming growth factor“-(TGF-)β-produzierenden regulatorischen T-Zellen dar. Des Weiteren kommt es zu einer Inhibition der Effektorzellen (Mastzellen, Basophile, Eosinophile, Neutrophile) und langfristig zu einer Verminderung der allergenspezifischen IgE-Produktion, während die allergenspezifische IgG4-Produktion ansteigt.
Summary
Besides the continuous improvement of the documentation of clinical efficacy of subcutaneous specific immunotherapy (SCIT) and, during the last years, of sublingual specific immunotherapy (SLIT), data concerning the immunologic mechanisms of specific immunotherapy (SIT) is increasingly leading to a better understanding of these treatments including their different indications and clinical effects (sustainability, preventive effect). The central explanation model of the immunologic mechanisms of SIT is a shift in the dominance of allergen-specific TH2 cells toward TH1 cells and the induction and activation or recruitment of IL-10- and transforming growth factor-(TGF-)β-producing regulatory T cells. Additionally, effector cells such as mast cells, basophils, eosinophils, and neutrophils are inhibited during SIT. These changes appear to be the reason for the inhibition of allergen-specific IgE production and the increase in allergen-specific IgG4 production.
