Zusammenfassung
Die chronische Urtikaria (CU) ist eine häufige Erkrankung, deren Therapie gelegentlich schwierig ist. Wenn die Identifikation und Elimination ursächlicher Auslöser nicht möglich sind, gibt es keine kausale Therapie. Symptomatische Standardtherapie der CU sind nicht sedierende H1-Antihistaminika (AH). Jedoch reicht die Wirkung einer Therapie mit AH bei einigen Patienten nicht aus, so dass die Urtikaria dann die Lebensqualität weiter stark beeinträchtigen kann. Dieser Artikel soll eine möglichst ausführliche Übersicht über Modalitäten zur Therapie der AH-refraktären CU geben. Der Einsatz von Ketotifen, Doxepin, H2-Rezeptor-blockern, Glukokortikoiden, Leukotrienrezeptor-antagonisten, COX-2-Inhibitoren, Antimalariamitteln, Dapson, Sulfasalazin, Antibiotika, Virostatika, Kalziumantagonisten, Danazol, ß-Sympathomimetika, Cumarin-Derivaten, Histaglobin, Ciclosporin, Tacrolimus, Methotrexat, Cyclophos-phamid und intravenösem Immunglobulin wird ebenso kritisch gewürdigt wie die Anwendung von Plasmapherese, Phototherapie, Diäten und psychosomatischer Therapie. Insgesamt finden sich zahlreiche Therapiealternativen zur Behandlung der CU beim Durchsuchen der medizinischen Fachliteratur. Viele therapeutische Ansätze sind jedoch nur auf einer niedrigen Evidenzstufe validiert. Zur besseren Bewertbarkeit neuer Therapieformen der CU sind kontrollierte klinische Studien zu fordern.
Summary
Chronic urticaria (CU) is a common disease. Control of the symptoms is sometimes difficult to achieve. If identification and elimination of underlying causes are not fruitful, there is no causal therapy. Non-sedating H1-antihistamines (AH) are the cornerstone of symptomatic treatment in CU. However, CU might be refractory to AH. In patients with unsatisfactory response to AH, urticaria can heavily impair their quality of life. The purpose of this article is to give a detailed overview of therapeutic modalities for treating patients with CU refractory to AH. Drugs such as ketotifen, doxepin, H2-blockers, glucocorticoids, leukotriene receptor antagonists, COX-2-inhibitors, antimalarials, dapsone, sulfasalazine, antibiotics, acyclovir, nifedipine, danazol, terbutaline, warfarin, histaglobin, cyclosporine, tacrolimus, methotrexate, cyclophosphamide and intravenous immunoglobulin are reviewed as well as measures such as plasmapheresis, phototherapy, diet and psychosomatic treatment. Overall, there are numerous alternative therapies for CU described in medical literature, but only limited evidence is available for many of them. For a more precise evaluation of the efficacy of novel therapies for CU controlled clinical studies are required in the future.
