Neue therapeutische Prinzipien bei Asthma bronchiale

Zusammenfassung

In den letzten zwei Jahren wurde eine Fülle von möglichen neuen Therapieansätzen evaluiert. Dabei handelt es sich einerseits um Weiterentwicklungen oder die Kombination von bekannten Substanzen, z.B. selektive Phosphodiesterase-4-Inhibitoren, neue Thromboxan-A₂-Rezeptor-Antagonisten, Steroide mit einer Plasmahalbwertszeit von wenigen Minuten (sog. Soft Steroids) oder die feste Kombination eines inhalativen Glukokortikosteroids mit einem lang wirksamen β₂-Agonisten. Andererseits konzentrierte sich die Forschung auf die Entwicklung spezifischer Inhibitoren der komplexen immunologischen Entzündungsantwort, wie z.B. Antizytokine, Antichemokine und Immunglobulinhemmer. Zusammenfassend ist damit zu rechnen, dass die Therapie des Asthma bronchiale in der Zukunft differenzierter und spezifischer erfolgen wird.

Summary

The past years have seen significant advances in the available and future treatments of asthma. The development of newer therapeutic agents can be divided into two groups. One includes advanced compounds or formulations which are already in use or which have been excessively tested in the past such as selective phosphodiesterase 4 inhibitors, novel thromboxane A₂ receptor antagonists, steroids with a half-life of less than 5 min (softs steroids) or combinations of inhalation therapies containing a fixed dose of a modern corticosteroid and a long acting β₂-agonist in one device. The other consists of compounds to inhibit specific parts of the complex immunologic inflammation process typically found in the bronchial tree of asthma patients. These are — among others — inhibitors of Th₂ cytokines, chemokine and immunoglobulin inhibitors. Thus, therapy of asthma will be more discriminative in the foreseeable future. This review gives a detailed overview of new trends in the pharmacotherapy of asthma.
