Zusammenfassung
Eine sichere Diagnose ist Voraussetzung für die erfolgreiche Beratung und Behandlung bei Nahrungsmittelallergie. Allergenkarenz gilt als Mittel der Wahl. Je nach Reaktion und Typ des verantwortlichen Nahrungsmittels, z. B. Reaktion auf essenzielles Grundnahrungsmittel, lebensbedrohliche Symptome gegen ein potenziell verstecktes Allergen oder ausschließlich orales Allergiesyndrom bei pollenassoziierter Kreuzreaktion auf Nahrungsmittel, ist der Betroffene differenziert zu beraten. Eine subkutane spezifische Immuntherapie mit Nahrungmittelallergenen (NMA) ist aufgrund der Risiken wissenschaftlichen Studien vorbehalten. Eine orale oder sublinguale Immuntherapie mit Nahrungsmittelallergenen kann derzeit nicht empfohlen werden. Pharmakotherapeutika sind vor allem im Notfall wichtig: Rasch resorbierbare Histamin-H1-Rezeptorantagonisten, rasch wirksame Glukokortikosteroide und bei Kreislaufreaktionen Adrenalin (zur Injektion oder Inhalation) gehören bei schweren Reaktionen auf Nahrungsmittel in die Notfallapotheke.
Summary
A firm diagnosis in case of food allergic reactions is a prerequisite for successful consulting and treatment. Allergen avoidance is the preferred option. Depending on the symptoms and the type of offending food allergen, i. e. symptoms to an essential basic food item, life-threatening symptoms to a potentially hidden food item, or exclusively oral allergy syndrome due to pollen-associated crossreactions to food allergens, differential advices should be given to the patient. Due to its risks sub-cutaneously applied specific immunotherapy with food allergens should only be performed in controlled trials. Oral and sublingual immunotherapy can not be recommended at present. Drugs for pharmacotherapeutical treatment become important in case of an emergency: Rapidly absorbed histamine-H1-blockers, glucocorticosteroids with early onset and adrenalin (for injection or inhalation) in case of anaphylactic cardiovascular symptoms are essentials of an emergency set for patients with severe reactions to foods.
