Diagnosi e terapia della malattia di Cushing in età pediatrica

Riassunto

La malattia di Cushing, causata da un eccesso di glucocorticoidi circolanti, rappresenta un’evenienza rara in età pediatrica e una sfida diagnostica e terapeutica per il pediatra. La diagnosi risulta spesso difficile perché nei bambini, a differenza dell’adulto, i segni e sintomi di sospetto della malattia di Cushing sono sfumati o non si manifestano per niente. È necessario, pertanto, sospettarla sempre in bambini che presentino un aumento di peso e/o un arresto della crescita. Anche il trattamento e il follow-up dei pazienti con questa patologia sono particolarmente critici. I pazienti pediatrici, infatti, anche dopo la cura, presentano problematiche legate all’ottimizzazione della crescita, alla mineralizzazione ossea, alla riproduzione e alla composizione corporea dall’infanzia alla vita adulta, necessitando di un continuo e attento follow-up.
