Summary
Although the clinical and functional importance of gliding and connective tissue spaces has been repeatedly emphasized (e.g. their role in the spreading of suppurative phlegmonic inflammation) only few literary findings can be presented dealing with the connective tissue spaces in the finger in the metacarpo-phalangeal transition region. Three separate gliding spaces of the finger above the dorsal aponeurosis and their various regional connections can be displayed by means of a plastic injection technique followed by plastination and production of sectional series. These gliding spaces were also examined on fixed and unfixed hands using plastic injection and subsequent dissection. A space was depicted between the proximal interphalangeal joint and the insertion of the dorsal aponeurosis on the distal phalanx of the finger, as well as a further bursa-like space over the proximal interphalangeal joint. A third space was also depicted between the metacarpo-phalangeal joint and the proximal interphalangeal joint, which displays a variable connection to the gliding canal of the respective extensor tendons. Methodical, functional and clinical aspects will be discussed.
Résumé
Même si l’importance clinique et fonctionnelle des espaces de glissement et des espaces conjonctifs a été soulignée de façon répétée (notamment leur rôle dans la diffusion des phlegmons), la littérature concernant les espaces conjonctifs des doigts dans la région métacarpo-phalangienne de transition est restreinte. Trois espaces de glissement séparés situés au-dessus de l’aponévrose dorsale et leurs diverses connexions régionales peuvent être décrits au moyen d’injections de résines plastiques, suivies de plastination et de coupes sériées. Ces espaces de glissement ont été examinés également sur des mains embaumées et sur des mains fraîches, après injections de résines plastiques et dissection. Un premier espace a été décrit entre l’art, interpha-langienne proximale (IPP) et l’insertion de l’aponévrose dorsale sur la phalange distale du doigt; un second espace en forme de bourse a été trouvé au-dessus de l’art, interphalangienne proximale, un troisième espace a également été décrit entre les art. métacarpo-phalangienne (MCP) et interphalangienne proximale (IPP), qui montre des connexions variables avec le canal de glissement du tendon extenseur correspondant. Les aspects méthodologiques, fonctionnels et cliniques sont discutés.
