Étonnante survie après méningite carcinomateuse d’origine probablement mammaire

Résumé

Les auteurs rapportent l’observation d’une femme présentant un cancer du sein agressif sans surexpression de HER 2 ni métastase. Trois mois après la fin de sa chimiothérapie adjuvante, elle a eu une méningite carcinomateuse documentée par la cytologie. Elle a reçu une nouvelle chimiothérapie par ifosfamide, doxoruribicine liposomale non pégylée et carboplatine puis une radiothérapie cérébrale. Alors que la survie à 2 ans est de 6% après une méningite carcinomateuse, cette femme est toujours en parfaite santé à 2 ans. Les auteurs mettent en avant le choix des médicaments passant la barrière hématoméningée pour expliquer cet excellent résultat. Notamment la doxoruribicine liposomale non pégylée qui a des concentrations intra-cérébrales et intra-tumorales supérieures à la doxorubicine.

Abstract

We report the case of a woman with an aggressive non metastatic HER 2 — breast cancer. She finished her adjuvant chemotherapy 3 months ago when we found a cytologic documentated carcinomatosis meningitis. She had a new chemotherapy with ifosfamid, non-pegylated liposomal doxorubicin and carboplatin and then brain radiotherapy. After such a meningitis, 2 year survival is 6% but she is in perfect health 2 years later. In order to explain these results, we choice 2 drugs which cross blood brain barrier and non-pegylated liposomal doxorubicin shows higher brain and intra tumor concentration than doxorubicin.
