Zusammenfassung
An der Universitätsaugenklinik in Wien werden seit 1993 additive Hinterkammerlinsen der Firma STAAR Surgical implantiert. Die Linsen werden als ICL (Intraokulare Kontaktlinse) bezeichnet. In die vorliegenden Untersuchungsergebnisse gehen 99 Augen von 58 Patienten ein. Die meisten Augen (89) waren höhergradig myop, der Rest hyperop. Die ICL-Modelle wurden im Beobachtungszeitraum mehrmals geändert, sodass keine einheitlichen Daten vorliegen. Die Anfangsergebnisse waren hervorragend, insbesonders bei hoher Myopie konnte durch die Operation die Lebensqualität sehr verbessert werden. Während des Beobachtungszeitraumes kam es zu keinem pathologischen Endothelzellverlust, der Augendruck blieb im Normbereich, es kam zu keinem Pigmentdispersionsglaukom. Als Komplikation traten aber im späteren Verlauf bei 25,5% Trübungen der kristallinen Linse auf. In 14,1% waren die subkapsularen vorderen Linsentrübungen progredient, und in 8,1% musste wegen ausgeprägter Visusverschlechterung eine Kataraktoperation mit Hinterkammerlinsenimplantation durchgeführt werden. Bisher sind die Ursachen dafür noch nicht eindeutig geklärt. Neue verbesserte ICL-Modelle, wie das Modell V4, mit stärkerer Wölbung soll den Abstand zwischen ICL und kristalliner Linse vergrößern und dadurch das Kataraktrisiko verringern. In Zukunft wird es auch notwendig sein, den Sulcus ciliaris-Durchmesser exakt zu vermessen, um eine gute Anpassung der Kunstlinse an das betroffene Auge zu ermöglichen. Dadurch sollten intermittierende oder ständige Berührungen zwischen kristalliner Linse und ICL vermieden werden können. Bisher fehlen noch Langzeitergebnisse mit den neuen, verbesserten Linsenmodellen. Da auch beim zuletzt verwendeten Modell V4 Linsentrübungen nicht ausgeschlossen werden können, kann die Verwendung noch nicht uneingeschränkt empfohlen werden.
Summary
Since 1993 we already have implanted 99 ICLs. The ICL-models changed during the follow-up period. Most of the eyes (89) were myopic, the rest were hyperopic. The early postoperative results were excellent, especially the high myopic patients achieved a significant increase in quality of life. No pathological endothelial cell loss could be found, intraocular pressure remained normal and no pigment dispersion glaucoma was observed. During the later follow-up we observed in 25.5% opacifications of the crystalline lens. In 14.1% they were progredient. In 8.1% a cataract operation with ICL implantation had to be performed. The reason for this complication is the aim of investigations. New ICL designs with more vaulting (V4) should improve long-term tolerance. In the near future it should be possible to measure the exact diameter of the ciliary sulcus area, because a permanent or temporary touch between ICL and crystalline lens, due to improper sizing, possibly contributes to cataract formation. As long as the cataract problem still exists, ICL implantation cannot be recommended without restriction.
