Zusammenfassung
Defokussierungskurven stellen ein einfaches Verfahren dar um die Schärfentiefe zu bestimmen. Hierbei werden unterschiedliche Objektabstände durch Vorhalten von Minuslinsen simuliert. Bei 15 Augen von 14 Patienten mit einer monofokalen Intraokularlinse wurde die Sehschärfe mit bester Fernkorrektur in einem Abstand von 6 m, 2 m, 1 m, 67 cm, 50 cm und 33 cm mit Hilfe von LogMAR-Tafeln ermittelt. Defokussierungskurven von −3,0 D bis −0,5 D wurden in 0,5 D Schritten, ausgehend von der besten Fernkorrektur, in einem Objektabstand von 6 m bestimmt. Die Ergebnisse der Visusbestimmung aus den angeführten Abständen waren besser als die Werte der Defokussierungskurve an allen Messpunkten. Die Unterschiede waren von 2 m (−0,5 D) bis 33 cm (−3,0 D) statistisch signifikant.
Schlussfolgerung
Ergebnisse von Defokussierungskurven berücksichtigen weder den Verkleinerungseffekt der vorgesetzten Minusgläser noch die eingeschränkte Objektdistanz von 6 m und die Miosis im Rahmen der Naheinstellung.
Summary
Aim of this investigation was to evaluate how visual acuities at different distances correlate with results from defocus curves. In 15 eyes (14 patients, age 67.9 years ± 9.7) with a monofocal silicone IOL (911 A, Pharmacia), best corrected distance visual acuity was determined at viewing distances of 6 m, 2 m, 1 m, 67 cm, 50 cm, 40 cm and 33 cm using LogMAR charts. Defocus curves were then evaluated from −3.0 D to −0.5 D at 0.5 D increments from the best distance correction at a viewing distance of 6 m. Distance visual acuity at 6 m was 1.23′ (minutes of arc) ± 0.21, at 2 m 1.28′ ± 0.5, at 1 m 1.55′ ± 0.47, at 67 cm 2.01′ ± 0.66, at 50 cm 2.30′ ± 0.85, at 40 cm 2.95′ ± 0.87 and at 33 cm 3.91′ ±1.18. Visual acuity decreased with defocusing with -0.5 D to 1.68′ ± 0.34, with −1.0 D to 2.37′ ± 0.62, with −1.5 D to 3.01′ ± 0.98, with −2.0 D to 3.96′ ± 1.31, with −2.5 D 5.44′ ± 1.52 and with −3.0 D to 6.57’ ± 1.69. Comparison of the visual acuity results from the two tests revealed statistically significant differences in the range from 2 m (−0.5 D) to 33 cm (−3.0 D).
Results of defocus curves disregard the diminutions of minus glasses, the normally restricted viewing distance to 6 m for distance acuity determination and in particular the physiological miosis due to the near point reaction.
