Zusammenfassung
Hintergrund
Transparente okuläre Gewebe wie Glaskörper und epiretinale Membranen sind während einer Vitrektomie häufig schwierig abzugrenzen. Durch Anfärben mit ICG lassen sich diese Strukturen besser darstellen.
Material und Methode
Nach Entfernen des zentralen Glaskörpers werden mit einer verdünnten ICG-Lösung die hintere Glaskörpergrenzschicht und der periphere Glaskörper angefärbt. Nach Induzieren einer hinteren Glaskörperabhebung und Abtragen der Glaskörperbasis können epiretinale Membranen durch erneute Eingabe von ICG angefärbt werden. Auch die sonst schlecht abgrenzbare Membrana limitans interna (ILM) ist so gut darstellbar.
Ergebnisse
An unserer Klinik wird seit einem Jahr in der Makulachirurgie ICG zum Anfärben der Membrana limitans interna verwendet. Indikationen für die Entfernung der ILM sind dabei Makulalöcher, das diabetische Makulaödem und zystoide Makulaödeme bei Uveitis. Durch die bessere Abgrenzbarkeit gegenüber der Netzhaut war bei allen Patienten ein unmittelbares Auffinden der ILM möglich. Bei dem anschließenden rhexisartigen Präparieren der ILM war stets die Grenze der Rhexis klar zu erkennen. Bei Unsicherheit über das Ausmaß des verbliebenen Glaskörpers bzw. einer vollständigen hinteren Glaskörperabhebung konnten diese durch eine verdünnte ICG-Lösung besser dargestellt werden.
Diskussion
ILM-Peeling kann durch Anwenden von ICG-Farbstoff wesentlich vereinfacht und damit sicherer gemacht werden. Ein zu tiefes Präparieren in die Netzhaut ist damit weitgehend ausgeschlossen. Durch den geringeren zeitlichen Aufwand ist mit weniger phototoxischen Reaktionen in der Makulachirurgie zu rechnen.
Summary
Background
Transparent intraocular tissues such as vitreous and epiretinal membranes are often difficult to differentiate intraoperatively. These structures can be better visualized using indocyaningreen (ICG).
Material and methods
Following a central core vitrectomy posterior vitreous membrane and peripheral vitreous are colored using a diluted ICG solution. After inducing a posterior vitreous detachment and removal of the vitreous base epiretinal membranes can be colored using ICG. Even a normally poorly visible internal limiting membrane (ILM) can be visualized very well.
Results
At our clinic ICG has been used in macular surgery for one year. Macular holes, diabetic macular edema, and macular edema due to uveitis were indications for ILM peeling. Because of the improved visualization the retina could be easily differentiated from the ILM. Especially the border of the ILM-rhexis was clearly defined during consecutive ILM preparation and peeling. If there was uncertainty regarding the amount of residual vitreous or the extent of a posterior vitreous detachment, the use of ICG made the surgical procedure much easier.
Discussion
ILM peeling becomes much easier and therefore safer by using ICG for a better visualization. Preparing too deeply into retinal tissue has thus become a rare complication. Because of an easier and faster surgical procedure the number of phototoxic reactions in macular surgery might be reduced as well.
