Zusammenfassung
Eine mögliche Anästhesiemethode bei Kataraktoperationen ist die intrakamerale Injektion von Lokalanästhetika. Die Endotheltoxizität dieser Substanzen bei Schweinehornhäuten wurde in vitro untersucht. Die Schweinehornhäute wurden in der Organkultur gelagert. Nachdem die Hornhäute 24 Stunden in dem Medium kultiviert wurden, wurde die Endothelzellzahl bestimmt. Hiernach wurden für eine Stunde je 10 Hornhäute in einer von drei verschiedenen Konzentrationen von Lidocain, Prilocain, Mepivacain und Bupivacain inkubiert und anschließend die Zellzahl erneut bestimmt und die Kultivierung fortgesetzt. Die Endothelzelldichte wurde im Laufe der Organkultur am dritten und am fünften Tag bestimmt. Eine Kontrollgruppe wurde ohne Inkubation in Anästhetika mit der gleichen Methode gelagert und befundet. Bei verschiedenen Konzentrationen von Lidocain (0,5 %, 1% und 2%) und Mepivacain (1%, 2% und 3%) konnte kein statistisch signifikanter Unterschied in Endothelzellverlust festgestellt werden. In der Gruppe von Prilocain (0,5%, 1% und 2%) deutete lediglich die 2%ige Lösung auf eine Schädigung hin, während Bupivacain (0,25%, 0,5% und 0,75%) zu einer signifikanten Abnahme der Endothelzellzahl im Vergleich zu der Kontrollgruppe führte. Aus diesem Grund schlussfolgern wird, dass eine intrakamerale Injektion von Lidocain, Mepivacain und Prilocain (0,5% und 1%) unbedenklich erscheint.
Summary
The intracameral injektion of topical anesthetics is a possible method of anesthesia for cataract surgery. The endothelial toxicity of these substances in pig corneas was examined in vitro. The pig corneas were stored with the method of organ culture. After 24 hours incubation the endothelial cell count was assessed. Then the corneas were incubated for one hour in three different concentrations of lidocaine, prilocaine, mepivacaine and bupivacaine. Afterwards the cells were counted and the corneas were stored again. During organ culture the cell count was assessed on 3rd and 5th day of subsequent culturing. A control group without incubation in anesthetics was treated in the same manner. Different concentrations of lidocaine (0.5%, 1%, 2%) and mepivacaine (1%, 2%, 3%) lead to no statistically significant differences in cell density. Among different concentrations of prilocaine (0.5%, 1% and 2%) just the 2% solution seemed to cause a cell damage. The 0.25%, 0.5% and 0.75% concentration of bupivacaine caused a significant cell reduction compared to the control group. The intracameral application of Lidocain (0.5%, 1%, 2%), Mepivacain (1%, 2%, 3%) and Prilocain (0.5%, 1%) seemed to be suitable for intracameral use.
