Zusammenfassung
In einer Reihe eigener Untersuchungen an enukleierten menschlichen Augen im Drucksystem mit offenem Hahn konnte die Hornhautrigidität (HR) mit Sicherheit erfasst werden: Sie stieg mit dem Druck in der Kammer an und sie fiel aus, wenn a) die Hornhaut mehrmals tief eingeschnitten, oder b) die Hornhaut durch Abtragen äußerer Schichten auf etwa die Hälfte verdünnt worden ist [4], aber auch c) während einer Applanation von 10 mm im Durchmesser [6]. In der gegenwärtigen Studie gelang der Nachweis, dass die zum Ausschalten der HR erforderliche Hh-Applanation (Y) vom Kammerdruck (X) linear abhängt: Der Gleichung für die Regressionsgerade Y = 5,34 + 0,02032 X ist zu entnehmen, dass pro Anstieg des X um 50 mm Hg, Y um 1,016 mm zunimmt, dass also eine Druckmessung erst dann frei vom Einfluss der HR ist, wenn die Hornhautapplanation bei 50 mm Hg 6,3, bei 200 mm Hg einen Durchmesser von 9,4 mm aufweist. Die klinische Anwendbarkeit derart großer Applanationen bedarf einer sorgfältigen Prüfung.
Summary
In previous studies the existence and the source of the corneal rigidity (CR) were clearly demonstrated: In open-stopcock-measurements on eviscerated human eyes, mounted upon a window of a transparent pressure-chamber, CR was almost linearly increasing with the pressure in the chamber up to 120 mm Hg, and the CR droped close to zero, when the cornea was deeply incised 3–4 times, or the cornea’s thickness was reduced to less than one half by ectomy of the outer layer [4] but also, when cornea was applanated up to 10 mm in diameter [6].
In the present study, at pressure-levels of 25 mm Hg-steps up to 200, it could be proved, that the diameter of the corneal applanation (Y), necessary to eliminate the CR shows with high significance (P = 0.0008) a linear dependence from the pressure in the chamber (X): The regression line Y = 5.3439 + 0.02032 derived means therefore: If you increase X by 50 mm Hg, the Y will increase by 1.016 mm, thus, the applanation diameter 6.5 mm at a pressure of 50 will increase to 9.4 mm at a pressure of 200 mm Hg. The clinical application of such large corneal applanations have to be throughly thougt over.
